What GLP-1 receptor agonists are

GLP-1 receptor agonists bind and activate the receptor for glucagon-like peptide-1, a natural hormone that helps regulate appetite and calorie intake and, in a glucose-dependent way, insulin release; labels describe GLP-1 receptors in brain areas involved in appetite regulation [3][6]. Semaglutide is a GLP-1 analogue with 94% sequence homology to human GLP-1 [3].

What the weight trials measured

Ozempic is approved for type 2 diabetes, not weight management [8]. Wegovy, Saxenda, generic liraglutide for weight, Zepbound and Foundayo have weight-management indications [3][4][5][6].

STEP 1 randomized 1,961 adults with obesity or overweight and no diabetes to weekly semaglutide 2.4 mg or placebo for 68 weeks: body weight fell 14.9% versus 2.4% [1]. SURMOUNT-1 enrolled 2,539 adults across four groups; weight fell 15.0%, 19.5% and 20.9% on tirzepatide 5, 10 and 15 mg versus 3.1% on placebo at 72 weeks [2]. The head-to-head weight trial, SURMOUNT-5, gave 751 adults tirzepatide or semaglutide at the maximum tolerated dose for 72 weeks: -20.2% versus -13.7% [7].

Tirzepatide is labeled as a GIP and GLP-1 receptor agonist, a dual agonist rather than a pure GLP-1 agonist [5].

What the labels warn about

Wegovy and Foundayo each carry a boxed warning about rodent thyroid C-cell tumors and are contraindicated in people with a personal or family history of medullary thyroid carcinoma or MEN 2; Foundayo's wording notes that orforglipron is not pharmacologically active in rodents and produced no rodent tumors [3][6]. With Wegovy 2.4 mg, nausea occurred in 44% versus 16% on placebo and vomiting in 24% versus 6%, and 6.8% versus 3.2% stopped treatment because of adverse reactions [3]. Labels also warn about pancreatitis, gallbladder disease, kidney injury from volume depletion, and hypoglycemia when the drug is combined with insulin or a sulfonylurea [3][6].

Why the comparison is uneven

The two sides rest on different levels of evidence. Semaglutide and tirzepatide have randomized human weight-loss trials [1][2]. 5-Amino-1MQ belongs to a class of methylquinolinium compounds that inhibit the enzyme NNMT; in a study of such inhibitors, they reduced body weight, fat mass and plasma cholesterol in high-fat-fed obese mice without changing food intake [9]. A PubMed search for 5-amino-1MQ returns research in cells and mice and no human weight-loss trial. A mouse result does not give a human weight change that can be set against the percentages above.
