AndroGel Rebound Effects When Stopping: What Happens to Your Testosterone

At a glance

  • Drug / AndroGel, a topical testosterone gel applied daily
  • Expected effect of stopping / loss of the added testosterone, not a surge above normal
  • Time course after the last application / Normal range for 24 to 48 hours, pretreatment levels by day 5 [8]
  • What sets your level afterward / how much testosterone your testes can produce
  • Why the cause matters / testicular failure versus a hypothalamus or pituitary problem [1]
  • Fertility / exogenous testosterone suppresses sperm production [2]
  • Sperm recovery in healthy men on hormonal contraception regimens / median 3.4 months to 20 million/mL [3]

Is a testosterone rebound after AndroGel real?

AndroGel is a topical testosterone gel applied to the skin daily. A rebound would mean testosterone climbs above normal once the gel stops. The expected change runs the other way: exogenous testosterone is no longer coming in, so the level falls back toward whatever the testes can make.

Testosterone from outside the body also suppresses the body's own reproductive hormone signalling. Exogenous testosterone suppresses intratesticular testosterone production, which is an absolute prerequisite for normal spermatogenesis [2].

What happens to your testosterone after you stop?

When AndroGel 1% treatment is discontinued after achieving steady state, the label says serum testosterone concentrations remain in the normal range for 24 to 48 hours but return to their pretreatment concentrations by the fifth day after the last application [8]. Once the last application has been absorbed and cleared, no more exogenous hormone is arriving. Where the level settles depends on why it was low in the first place. FDA describes the conditions testosterone products are approved to treat as either failure of the testes to produce testosterone or a problem in the hypothalamus or pituitary, the brain structures that control production by the testes [1].

When the testes do not make enough testosterone, levels may remain low after AndroGel stops. If the problem involves the hypothalamus or pituitary, levels also depend on whether those signals recover [1].

Sperm and fertility after stopping AndroGel

Sperm production is a separate question from serum testosterone, and it is where recovery has actually been measured. Exogenous testosterone suppresses intratesticular testosterone production, which normal spermatogenesis requires [2]. The Endocrine Society recommends against starting testosterone therapy in men planning fertility in the near term [4].

The best recovery data come from healthy men on testosterone-based hormonal contraception regimens, not from men using AndroGel. Across 30 studies and 1,549 men, the median time for sperm concentration to return to 20 million/mL was 3.4 months; 67% had recovered within 6 months, 90% within 12, 96% within 16 and all within 24 months, with faster recovery in older men, after shorter treatment and with shorter-acting testosterone [3].

For men who developed infertility while on testosterone, recovery has been studied with treatment. In a retrospective study of 66 men who stopped testosterone and received hCG plus a selective estrogen receptor modulator, 46 men (70%) reached a total motile sperm count above 5 million within 12 months [5]. Older age and longer testosterone use predicted slower recovery [5]. In a separate case series of 49 men with azoospermia or severe oligospermia while on testosterone, treatment with hCG plus clomiphene, tamoxifen, anastrozole or recombinant FSH returned or improved sperm production in 47 men (95.9%), after an average of 4.6 months [6].

A review of anabolic steroid-induced hypogonadism lists testosterone replacement, hCG and selective estrogen receptor modulators as management options, with the choice depending on the dose, duration and type of steroid used [7].

Frequently asked questions

Does stopping AndroGel cause a testosterone rebound?
A surge above normal is not the expected effect. Once the gel stops, no more exogenous testosterone is arriving, so the level falls back toward what the testes can produce, which in men whose testes fail may be low [1].
How long until my own testosterone returns after stopping?
The AndroGel 1% label says serum testosterone stays in the normal range for 24 to 48 hours after stopping at steady state and is back to pretreatment concentrations by the fifth day after the last application [8]. Where it settles depends on the cause. When the testes do not make enough testosterone, levels may remain low after AndroGel stops. If the problem involves the hypothalamus or pituitary, levels also depend on whether those signals recover [1].
Will sperm count recover after stopping AndroGel?
Exogenous testosterone suppresses intratesticular testosterone production, which normal spermatogenesis requires [2]. Among healthy men on hormonal contraception regimens, sperm concentration returned to 20 million/mL at a median of 3.4 months, with 90% recovered within 12 months [3]. In 66 men with infertility after testosterone use who stopped it and received hCG plus a selective estrogen receptor modulator, 70% reached a total motile count above 5 million within 12 months [5].
Can hCG or clomiphene help after stopping AndroGel?
They have been studied in men with infertility after testosterone use, not specifically after AndroGel. In a case series of 49 men on testosterone, hCG 3,000 units subcutaneously every other day plus clomiphene, tamoxifen, anastrozole or recombinant FSH restored or improved sperm production in 47 men, average 4.6 months [6].
What determines my testosterone level once AndroGel is stopped?
Whether the testes can produce testosterone, and whether the hypothalamus and pituitary signal recovers. FDA describes the approved conditions as testicular failure or a hypothalamus or pituitary problem [1].
