Can I Stop Semaglutide Cold Turkey? A Clinical Answer

At a glance

  • Withdrawal syndrome / Not listed as a warning on the Ozempic, Wegovy or Rybelsus labels [1][2][3]
  • Half-life / About 1 week; semaglutide stays in the body roughly 5 to 7 weeks after the last 2.4 mg or 7.2 mg dose [3]
  • STEP 4, week 20 to 68 / Body weight +6.9% on placebo vs -7.9% continuing semaglutide 2.4 mg [4]
  • STEP 1 extension, one year off drug / 11.6 percentage points of the 17.3% lost were regained; net loss 5.6% vs 0.1% on placebo [5]
  • Taper schedule / The Wegovy and Ozempic labels give none; their dosing covers escalation and missed doses [2][3]
  • Planned pregnancy / The Wegovy and Ozempic labels say to stop at least 2 months beforehand [2][3]

The short answer

The three current US labels describe reactions that occur while taking the drug (nausea, vomiting, diarrhea, constipation, abdominal pain, gallbladder disease, pancreatitis, kidney injury from dehydration, hypersensitivity and a thyroid C-cell tumor warning, among others), and none of them describes a discontinuation or withdrawal syndrome [1][2][3].

Weight regain followed stopping in the trials below.

How long semaglutide stays in the body

The Wegovy label gives semaglutide a half-life of about one week and states that the drug remains in the body for roughly 5 to 7 weeks after the last 2.4 mg or 7.2 mg injection or 25 mg tablet [3]. It is cleared by normal protein breakdown rather than by liver enzymes, and about 3% of a dose is excreted unchanged in urine [3].

What the trials show about weight after stopping

STEP 4 was a withdrawal trial in which participants either continued semaglutide or switched to placebo. Nine hundred and two adults with obesity or overweight, without diabetes, started semaglutide and escalated to 2.4 mg weekly over 20 weeks [4]. The 803 who reached that dose (mean weight loss 10.6% by week 20) were then randomized, two to one, to keep taking semaglutide (535 people) or switch to placebo (268 people) for 48 more weeks, both groups with lifestyle intervention [4].

From week 20 to week 68, the continuing group lost another 7.9% of body weight. The placebo group gained 6.9%. The difference between arms was 14.8 percentage points [4].

A second set of numbers comes from an exploratory off-treatment extension of STEP 1, which is smaller and less definitive. It followed 327 participants for a year after treatment stopped [5]. Mean weight loss at week 68 was 17.3% on semaglutide versus 2.0% on placebo; by week 120, the semaglutide group had regained 11.6 percentage points, leaving a net loss of 5.6% from baseline compared with 0.1% for placebo [5]. Most of the cardiometabolic improvements seen during treatment moved back toward baseline over that year [5].

In both trials, participants who stopped semaglutide regained weight; in the STEP 1 extension the regain was about two-thirds of the weight lost, not a full return to starting weight [5].

Does the label give a taper schedule?

The Wegovy label's dosing section is built around starting and escalating: 0.25 mg for weeks 1 to 4, 0.5 mg for weeks 5 to 8, 1 mg for weeks 9 to 12, 1.7 mg for weeks 13 to 16, then maintenance from week 17 at 2.4 mg or 1.7 mg [3]. Its instructions for interrupted treatment concern missed doses, not a step-down schedule for stopping: give the missed dose if the next one is more than 2 days away, and after 2 or more consecutive missed doses, re-escalate from a lower dose [3]. The Ozempic label says to give a missed weekly dose within 5 days, otherwise skip it [2]. Neither label sets out a separate schedule for a break of several months.

Planned surgery or deep sedation

The labels report pulmonary aspiration in patients receiving GLP-1 receptor agonists who underwent elective surgeries or procedures under general anesthesia or deep sedation, and say patients should inform their healthcare providers of any planned surgeries or procedures [2][3].

Pregnancy and a planned stop

The Wegovy label states that weight loss offers no benefit in pregnancy and may cause fetal harm, that Wegovy should be discontinued when pregnancy is recognized if it is being used for weight reduction, and that it should be stopped at least 2 months before a planned pregnancy because of the long half-life [3]. The Ozempic label also says to stop at least 2 months before a planned pregnancy [2].

If the semaglutide came from a compounding pharmacy

The STEP 1 and STEP 4 findings apply to the semaglutide products and study populations tested in those trials; they do not automatically establish the same results for compounded versions [5][4]. FDA says compounded drugs are not FDA approved and are not reviewed for safety, effectiveness or quality before marketing [6].

Frequently asked questions

Frequently asked questions

Do you have to taper off semaglutide?
The US labels do not provide a taper schedule for stopping semaglutide; their dosing sections cover escalation and missed doses [2][3].
How long does semaglutide stay in your system after the last dose?
The Wegovy label gives a half-life of about one week and says semaglutide remains in the body for roughly 5 to 7 weeks after the last 2.4 mg or 7.2 mg injection or 25 mg tablet [3].
Will I regain weight if I stop?
Partly, in the trial data. In STEP 4, people switched to placebo after 20 weeks gained 6.9% of body weight by week 68 while those who continued lost another 7.9% [4]. In the STEP 1 extension, participants regained 11.6 of the 17.3 percentage points they had lost, for a net loss of 5.6% one year after stopping [5].
Can I restart at my old dose after a break?
For Wegovy injections, the label calls for re-escalation from a lower dose after two or more consecutive missed doses [3]. It does not set out a separate schedule for gaps of several months.
Is stopping cold turkey dangerous?
The current labels list no withdrawal syndrome [1][2][3]. They do name events that call for discontinuation: the Ozempic label says to discontinue if pancreatitis is suspected [2], and the EMA advises stopping semaglutide if NAION, a very rare eye side effect it estimates at up to 1 in 10,000 people, is confirmed [7].
