Maximum Dose of Ozempic for Weight Loss

At a glance

  • Ozempic maximum dose / 2 mg once weekly [1]
  • Ozempic weight-loss indication / none on the label [1]
  • Wegovy recommended maintenance dose / 2.4 mg once weekly [10]
  • Wegovy adult maximum / 7.2 mg once weekly [10]
  • Ozempic tablets maximum / 9 mg once daily, diabetes only [3]
  • Wegovy tablets maximum / 25 mg once daily, adults [10]
  • Highest weekly dose tested for weight loss / 7.2 mg in STEP UP [5]

Does Ozempic have a weight-loss dose at all?

No. The Ozempic label lists three uses, all in adults with type 2 diabetes: improving glycemic control alongside diet and exercise; reducing the risk of major adverse cardiovascular events in people with established cardiovascular disease; and reducing the risk of sustained eGFR decline, end-stage kidney disease and cardiovascular death in people with chronic kidney disease [1].

The weight-management indication for semaglutide belongs to Wegovy, a separate FDA-approved product [10]. The highest Ozempic dose and the highest semaglutide dose for weight loss are two different numbers from two different labels.

What is the maximum dose of Ozempic?

2 mg once weekly by subcutaneous injection [1].

The label's escalation is 0.25 mg once weekly for 4 weeks - a starting dose, not a maintenance dose - then 0.5 mg. If more glycemic control is needed after at least 4 weeks on 0.5 mg, the dose can go to 1 mg, and after at least 4 weeks on 1 mg it can go to 2 mg [1]. The label says to follow that dosage escalation to reduce the risk of gastrointestinal adverse reactions [1]. For the kidney indication, the maintenance dose is 1 mg, reached after at least 4 weeks on 0.5 mg [1].

On missed doses, the Ozempic instructions say a dose can be given within 5 days of the missed dose; after 5 days, it is skipped and the next dose taken on the regularly scheduled day [1].

Ozempic tablets are a separate form with a separate ceiling: 1.5 mg daily for 30 days, then 4 mg, then 4 mg or 9 mg from day 61, for type 2 diabetes only [3]. Wegovy tablets are a separate product, at a maximum of 25 mg once daily for adults [10][4].

So what is the maximum semaglutide dose for weight loss?

Wegovy. The labeled schedule steps up through 0.25 mg, 0.5 mg, 1 mg and 1.7 mg before a recommended maintenance dose of 2.4 mg once weekly [10]. The adult maximum is 7.2 mg once weekly, for adults who have tolerated 2.4 mg for at least 4 weeks and need more weight loss [10]. Drugs@FDA lists Wegovy injection strengths of 0.25, 0.5, 1, 1.7 and 2.4 mg, plus a 7.2 mg strength (Wegovy HD) [2].

The two landmark weight-loss numbers behind those ceilings:

  • STEP 1: 1,961 adults with obesity, or overweight with a weight-related condition, and no diabetes, treated for 68 weeks. Mean weight change was -14.9% with semaglutide 2.4 mg weekly versus -2.4% with placebo, a difference of 12.4 percentage points [6].
  • STEP UP: 1,407 adults with obesity and no diabetes, treated for 72 weeks. Mean weight change was -18.7% on 7.2 mg versus -15.6% on 2.4 mg versus -3.9% on placebo [5].

Does the higher maximum produce more weight loss?

A little more, on average. In STEP UP, 7.2 mg produced 3.1 percentage points more weight loss than 2.4 mg [5]. The dose response shows up in the diabetes formulation too: in SUSTAIN FORTE, 961 adults with type 2 diabetes on metformin lost 6.9 kg on semaglutide 2 mg versus 6.0 kg on 1 mg over 40 weeks [7].

Weight loss is smaller in people with type 2 diabetes than in people without it. In STEP 2, 1,210 adults with type 2 diabetes and a BMI of 27 or more lost 9.6% of body weight on 2.4 mg versus 3.4% on placebo at 68 weeks, a difference of 6.2 percentage points [8] - well below the 14.9% seen without diabetes in STEP 1 [6].

What does the label say to watch for at higher doses?

Gastrointestinal reactions track with dose in the Ozempic program. In pooled placebo-controlled trials in type 2 diabetes, nausea occurred in 6.1% on placebo, 15.8% on 0.5 mg and 20.3% on 1 mg; vomiting in 2.3%, 5% and 9.2%; diarrhea in 1.9%, 8.5% and 8.8% [1]. Any GI reaction occurred in 15.3%, 32.7% and 36.4% respectively, and 0.4%, 3.1% and 3.8% stopped treatment for GI reasons [1]. In the 40-week trial comparing 2 mg with 1 mg in 959 patients, GI reactions were 34% on 2 mg versus 30.8% on 1 mg [1].

Two items apply at every dose. Ozempic carries a boxed warning about thyroid C-cell tumors seen in rodents, with human relevance unknown, and is contraindicated in people with a personal or family history of medullary thyroid carcinoma or MEN 2 [1]. The label also says an Ozempic pen must never be shared between patients, even if the needle is changed [1].

The warnings section lists acute pancreatitis (the label says to discontinue if it is suspected), acute gallbladder disease, hypoglycemia when combined with insulin or a sulfonylurea, acute kidney injury from volume depletion, severe GI reactions, hypersensitivity reactions including anaphylaxis and angioedema, and pulmonary aspiration during general anesthesia or deep sedation [1]. The label states Ozempic is not recommended in patients with severe gastroparesis [1].

What FDA says about compounded semaglutide dosing

FDA reports dosing errors with compounded injectable semaglutide, including patients measuring the wrong dose, clinicians miscalculating, and doses above the label [9]. Compounded drugs are not FDA-approved and are not reviewed for safety, effectiveness or quality before marketing, and FDA had received 990 adverse event reports for compounded semaglutide as of 2026-05-31, a figure it says likely undercounts [9].

Frequently asked questions

What is the maximum dose of Ozempic?
2 mg once weekly by subcutaneous injection, reached after at least 4 weeks on 1 mg if more glycemic control is needed [1]. The 0.25 mg starting dose is not a maintenance dose [1].
Can Ozempic be prescribed for weight loss?
The Ozempic label has no weight-loss indication at any dose; it covers glycemic control, cardiovascular risk reduction with established cardiovascular disease, and kidney outcomes with chronic kidney disease in adults with type 2 diabetes [1].
Is 2.4 mg the highest semaglutide dose for weight loss?
No. Wegovy recommends 2.4 mg once weekly and allows up to 7.2 mg weekly in adults who tolerate 2.4 mg for at least 4 weeks and need more weight loss [10]. In STEP UP, 7.2 mg produced a mean weight change of -18.7% versus -15.6% on 2.4 mg over 72 weeks [5].
How long does it take to reach the maximum Wegovy dose?
The labeled schedule uses four-week steps at 0.25, 0.5, 1 and 1.7 mg, so 2.4 mg is the maintenance dose from week 17 [10]. The label allows the 7.2 mg maximum for adults who have tolerated 2.4 mg for at least 4 weeks and need more weight loss [10].
Do higher doses cause more side effects?
GI reactions rose with dose in the Ozempic trials: any GI reaction in 15.3% on placebo, 32.7% on 0.5 mg and 36.4% on 1 mg, and 34% on 2 mg versus 30.8% on 1 mg in a separate 40-week comparison of 959 patients [1].
What happens if an Ozempic dose is missed?
The label says to give the dose within 5 days of the missed dose; if more than 5 days have passed, skip it and take the next dose on the regularly scheduled day [1].
