What the label says about tadalafil and bariatric surgery

Cialis (tadalafil) is a PDE5 inhibitor approved for erectile dysfunction, for the signs and symptoms of benign prostatic hyperplasia, and for the two conditions together; the label currently in force is dated June 3, 2026 [1]. Drugs@FDA lists the 5, 10 and 20 mg tablets as prescription products and the 2.5 mg tablet as discontinued [2].

The label sets its doses by indication, kidney function, liver function and interacting drugs, and its dosing section does not mention bariatric surgery [1]. The label does not say how gastric bypass or sleeve gastrectomy changes tadalafil levels. What follows is what the label does say, plus one example of how gastric bypass changed another drug's absorption.

Does bariatric surgery change how tadalafil is absorbed?

The Cialis label reports these measurements: peak tadalafil concentration between 30 minutes and 6 hours after a dose (median 2 hours), a mean terminal half-life of 17.5 hours, and rate and extent of absorption that are not influenced by food [1]. Tadalafil is metabolized mainly by CYP3A4; the metabolites are not expected to be pharmacologically active, and about 61% of a dose leaves in feces and 36% in urine as metabolites [1]. Once-daily dosing reaches steady state within 5 days, at about 1.6 times the exposure of a single dose [1].

Bariatric operations can change the absorption of some oral drugs. In a single-dose study, 16 nondiabetic patients who had undergone gastric bypass were compared with 16 matched controls: metformin exposure was 21% higher (13.7 vs 11.4 micrograms per mL per hour; the 95% CI of the difference, -1.3 to 5.9, included zero) and bioavailability 50% higher (41.8% vs 27.8%) [3]. Those figures are metformin's, not tadalafil's.

What the Cialis label lists for dosing

The Cialis label lists 10 mg before sexual activity as the usual starting dose for as-needed ED treatment, with a 5 to 20 mg range based on response and tolerability and a maximum frequency of once daily [1]. It reports improved erectile function compared with placebo for up to 36 hours after a dose; that does not mean a continuous erection, because sexual stimulation is still required [1].

For once-daily ED, the label's starting dose is 2.5 mg at about the same time every day, which may be increased to 5 mg [1]. For BPH, or for ED with BPH, it lists 5 mg once daily at about the same time every day; when Cialis is started together with finasteride for BPH, the label recommends that use for up to 26 weeks [1]. The label says not to split the tablets and that the entire dose should be taken [1].

Kidney, liver and interaction limits in the label

At a creatinine clearance of 30 to 50 mL/min, the label's as-needed regimen starts at 5 mg and is capped at 10 mg no more often than every 48 hours; below 30 mL/min or on hemodialysis the maximum is 5 mg no more often than every 72 hours, and once-daily use is not recommended [1]. For BPH or ED with BPH at 30 to 50 mL/min, it starts at 2.5 mg and allows an increase to 5 mg [1].

With Child-Pugh class A or B, the label says as-needed dosing should not exceed 10 mg once a day; in class C, Cialis is not recommended [1].

CYP3A4 drugs change exposure substantially. Ketoconazole 400 mg daily raised tadalafil 20 mg exposure (AUC) by 312% and peak concentration by 22%, and ketoconazole 200 mg daily raised tadalafil 10 mg AUC by 107% [1]. Ritonavir 200 mg twice daily raised AUC by 124%; higher ritonavir doses raised AUC by 32% while lowering peak concentration by 30% [1]. Rifampin 600 mg daily cut tadalafil 10 mg AUC by 88% and peak concentration by 46% [1]. With a potent CYP3A4 inhibitor, the label caps as-needed Cialis at 10 mg every 72 hours and once-daily Cialis at 2.5 mg [1].

Alpha-blockers lower blood pressure too. In healthy volunteers, tadalafil 20 mg with doxazosin 8 mg daily produced a placebo-subtracted maximal standing systolic fall of 9.8 mm Hg [1]. In a second doxazosin study, 16 of 24 men had a systolic reading below 85 mm Hg or a fall over 30 mm Hg after tadalafil, against 6 on placebo [1]. With tamsulosin 0.4 mg the standing fall was 1.7 mm Hg with tadalafil 10 mg and 2.3 mm Hg with 20 mg, and 5 mg daily plus tamsulosin produced no outliers [1]. For ED, the label says to establish the alpha-blocker first and start tadalafil at the lowest dose; for BPH it does not recommend the combination, because efficacy has not been adequately studied and blood pressure can drop [1].

What the label says to watch for

Nitrates: the label contraindicates any organic nitrate, taken regularly or intermittently [1]. In a life-threatening situation where a nitrate is medically necessary, it says at least 48 hours should pass after the last Cialis dose, with close medical supervision and hemodynamic monitoring [1]. In a 150-man crossover study, tadalafil 20 mg daily for 7 days followed by sublingual nitroglycerin 0.4 mg pushed standing systolic pressure below 85 mm Hg in more men at 4, 8 and 24 hours, with no difference at 48 hours and beyond [4]. The label adds that at 48 hours a few more tadalafil men still had greater blood pressure falls [1].

Riociguat and other guanylate cyclase stimulators are contraindicated [1].

Cardiovascular: the label says ED drugs should not be used in men for whom sexual activity is inadvisable because of cardiovascular status, and it does not recommend Cialis in several groups that were not studied, including myocardial infarction within 90 days, unstable angina or angina during intercourse, NYHA class 2 or greater heart failure within 6 months, uncontrolled arrhythmia, blood pressure below 90/50 mm Hg or uncontrolled hypertension, and stroke within 6 months [1]. In healthy volunteers, tadalafil 20 mg lowered supine blood pressure by a mean maximal 1.6/0.8 mm Hg compared with placebo [1].

Priapism: the label says an erection lasting more than 4 hours, painful or not, needs emergency treatment [1].

Eyes and ears: the label says to stop the drug and seek medical attention for sudden vision loss in one or both eyes, which can be non-arteritic anterior ischemic optic neuropathy, and for sudden decrease or loss of hearing [1].

Alcohol: the label says five units or more with Cialis can increase orthostatic symptoms such as a faster heart rate, lower standing blood pressure, dizziness and headache, and that alcohol and tadalafil do not change each other's blood levels [1].

Adcirca is a separate tadalafil product, 20 mg tablets approved for pulmonary arterial hypertension rather than for ED [5]. Both Cialis and Adcirca are tadalafil, and the Cialis label says combinations with other ED treatments have not been studied [1].

Testosterone after bariatric surgery

In a study of 33 men with a mean body mass index of 50.3 who underwent bariatric surgery, 78.8% had hypogonadism before surgery, defined as total testosterone below 300 ng/dL; after 12 months, with mean weight loss of 18.8%, persistent hypogonadism was 6% by total testosterone and 15% by free testosterone [6]. The Cialis label reports no adverse effect of tadalafil 10 or 20 mg on mean testosterone, luteinizing hormone or follicle-stimulating hormone concentrations compared with placebo [1].

Frequently asked questions

Can I take Cialis after gastric bypass surgery?
The Cialis label does not address bariatric surgery. Its contraindications are any organic nitrate, guanylate cyclase stimulators such as riociguat, and a history of serious hypersensitivity to tadalafil [1]. Its doses are 10 mg as needed for ED (range 5 to 20 mg, once daily maximum), 2.5 mg once daily for ED increasing to 5 mg, and 5 mg once daily for BPH or ED with BPH [1].
What dose of tadalafil does the label give?
For ED as needed, 10 mg before sexual activity, adjustable between 5 and 20 mg, no more than once a day. For daily-use ED, 2.5 mg, possibly 5 mg. For BPH or ED with BPH, 5 mg once daily [1].
Can I split Cialis tablets?
The Cialis label says not to split the tablets and that the entire dose should be taken [1].
Can I take Cialis with tamsulosin after bariatric surgery?
Both drugs lower blood pressure. With tamsulosin 0.4 mg, the standing systolic fall was 1.7 mm Hg with tadalafil 10 mg and 2.3 mm Hg with 20 mg, and 5 mg daily plus tamsulosin produced no outliers [1]. For ED the label says to be stable on the alpha-blocker first and start tadalafil at the lowest dose; for BPH it does not recommend the combination [1].
Does gastric bypass change how drugs are absorbed?
It can for some drugs. In a single-dose study, metformin bioavailability was 50% higher after gastric bypass than in matched controls (41.8% vs 27.8%) [3]. That figure is metformin's; the Cialis label does not give tadalafil data after bariatric surgery [1].
Is Cialis ever acceptable with nitroglycerin?
Organic nitrates are contraindicated with Cialis [1]. In a life-threatening situation the label says at least 48 hours should pass after the last Cialis dose before a nitrate is considered, under close supervision with hemodynamic monitoring [1]. In a 150-man study the blood pressure interaction was significant up to 24 hours [4].
