Triple Agonists (GLP-1/GIP/Glucagon) Billing & Prior-Auth Playbook

At a glance

  • FDA-approved triple agonists / none found; no FDA-approved retatrutide product [1]
  • Retatrutide status / not FDA approved; cannot be used in compounding under federal law [1]
  • Approved product that activates both GIP and GLP-1 receptors / tirzepatide, a dual agonist (Mounjaro, Zepbound) [2][3]
  • Approved GLP-1 products for weight / Wegovy injection and tablets, Saxenda, generic liraglutide, Zepbound, Foundayo [4][5][2][6]
  • Contraindication on boxed-warning GLP-1 labels / personal or family history of medullary thyroid carcinoma or MEN 2 [4][6]

Is any GLP-1/GIP/glucagon triple agonist FDA approved?

No FDA-approved retatrutide product was found. FDA states that retatrutide cannot be used in compounding under federal law, is not a component of an FDA-approved drug, and has not been found safe and effective for any condition [1]. The agency has warned telehealth companies for marketing unapproved drugs such as retatrutide, ingredient distributors for selling it to compounders, and outsourcing facilities for repackaging it [1]. Compounded drugs are not FDA approved and are not reviewed for safety, effectiveness or quality before they are marketed [1].

Which approved products have labeled indications

If the clinical goal is a GLP-1-based weight or diabetes drug, the approved alternatives have their own labeled indications, which differ by product:

  • Wegovy injection: weight reduction and long-term maintenance in adults and children 12 and older with obesity, or adults with overweight and a weight-related condition; cardiovascular risk reduction in adults with established cardiovascular disease and obesity or overweight; and noncirrhotic MASH with moderate to advanced fibrosis in adults [4].
  • Wegovy tablets: weight reduction and cardiovascular risk reduction in adults [4].
  • Saxenda: weight management in adults with obesity or overweight plus a weight-related condition, and in children 12 and older weighing over 60 kg with obesity [5].
  • Zepbound: weight management and moderate to severe obstructive sleep apnea in adults with obesity [2].
  • Foundayo (orforglipron): an oral GLP-1 receptor agonist approved for weight reduction and long-term maintenance in adults with obesity, or with overweight and at least one weight-related condition [6].

Mounjaro and Zepbound contain tirzepatide, which the label describes as a GIP and GLP-1 receptor agonist: a dual agonist, not a triple agonist [2][3]. The Wegovy, Saxenda, Zepbound and Foundayo labels say that use with another GLP-1 receptor agonist is not recommended [4][5][2][6].

What the labels constrain in a request

The labeled population is not worded the same way across products. Wegovy injection covers adults and children 12 and older with obesity, while the Wegovy tablet is approved in adults [4]. Saxenda adds children 12 and older weighing over 60 kg with obesity to its adult population [5]. Foundayo's label has no diabetes indication [6]. Wegovy's MASH indication is approved under accelerated approval based on improvement of MASH and fibrosis, and continued approval may be contingent on a confirmatory trial [4].

Frequently asked questions

Is there a prior-authorization pathway for retatrutide or another triple agonist?
No FDA-approved retatrutide product was found, and FDA states retatrutide has not been found safe and effective for any condition [1]. Without an approved product or labeled indication there is nothing to authorize against; coverage rules are set by each plan.
What is the approved product that activates both GIP and GLP-1 receptors?
Tirzepatide (Mounjaro for type 2 diabetes, Zepbound for weight and sleep apnea) is labeled as a GIP and GLP-1 receptor agonist, a dual agonist rather than a triple agonist [2][3]. Wegovy, Saxenda and Foundayo are single GLP-1 receptor agonists [4][5][6].
Does an approved GLP-1 drug have a labeled indication to match against?
Yes. Each approved label lists its own indications, for example Zepbound for weight management and obstructive sleep apnea [2] and Wegovy injection for weight reduction, cardiovascular risk reduction and MASH [4].
