- Testicular effect / Leydig cells make more testosterone; Sertoli cells support sperm [1]
- Best-fit condition / secondary (hypogonadotropic) hypogonadism [1]
- Not expected to work in / primary hypogonadism (testicular failure) [1]
- Approval / not FDA-approved; EMA refused EnCyzix in 2018 [1][2]

What does enclomiphene do in the body?
Enclomiphene does not add testosterone. It lifts estrogen's suppression of the pituitary. Estrogen normally suppresses the pituitary-testis axis, and enclomiphene blocks estrogen receptors in the pituitary [1]. With that suppression reduced, the pituitary releases more LH and FSH [1]. LH then signals Leydig cells in the testes to make testosterone. FSH supports Sertoli cells, which are needed for sperm production [1]. The result is the body's own testosterone, not an external testosterone dose. The phase 3 report describes this as restoration instead of replacement [3]. Enclomiphene is not a testosterone product, and its effect depends on a pituitary that can raise LH and FSH and on testes that can respond [1].
What happens to LH, FSH, and sperm?
In the phase 3 ZA-304 and ZA-305 studies, total testosterone rose in all treatment groups over 16 weeks [3]. LH and FSH rose with enclomiphene and fell with testosterone gel [3]. Sperm concentration stayed in the normal range on enclomiphene, while there was a marked reduction in spermatogenesis on testosterone gel [3]. That is the mechanistic contrast: exogenous testosterone suppresses LH and FSH; enclomiphene raises them. The sperm effect is not absolute. In ZA-301, sperm concentration fell by 50% or more in 14.2% of men on enclomiphene versus 2.6% on placebo [4]. In ZA-302, the figures were 14.2% versus 4.3% [5]. So enclomiphene preserved sperm better than testosterone gel in these trials, but some men still had a large decline.
What did the phase 3 trials actually measure?
ZA-301 enrolled 151 overweight men aged 18 to 60 with acquired hypogonadotropic hypogonadism, morning testosterone under 300 ng/dL and normal sperm concentration; men received enclomiphene 12.5 mg or 25 mg daily or placebo for 12 weeks [4]. Among pooled enclomiphene patients, 78.8% reached an average 24-hour testosterone in the normal range of 300 to 1,040 ng/dL (95% CI 70.3 to 85.3, n=113) [4]. ZA-302 enrolled 181 men, and 81.3% of those on enclomiphene reached the normal range (95% CI 73.9 to 87.0, n=134) [5].These trials measured hormones and sperm counts. Enclomiphene raised testosterone in these studies, but they did not establish whether symptoms improved [1].
How is enclomiphene different from clomiphene?
Clomiphene citrate is a mixture of two isomers: enclomiphene (trans) and zuclomiphene (cis) [1]. Per FDA, the USP monograph gives enclomiphene content of clomiphene as 50 to 70 percent [1]. They are related but not the same drug. Zuclomiphene is more estrogenic; enclomiphene is the more anti-estrogenic, pro-gonadotropic isomer [6]. The clomiphene label says zuclomiphene has a longer half-life than enclomiphene and that detectable levels persisted for longer than a month [7].Clomiphene itself is FDA-approved only for ovulatory dysfunction in women desiring pregnancy [7]. Its label has no male indication and says there are no adequate or well-controlled studies demonstrating effectiveness in male infertility; testicular tumors and gynecomastia have been reported in males using clomiphene, with cause and effect not known [7]. Male use of clomiphene for low testosterone is off-label [7]. Enclomiphene is not interchangeable with clomiphene.
A retrospective study of 66 men who took clomiphene and later enclomiphene found a median testosterone rise of 166 versus 98 ng/dL (P=0.20) and an estradiol change of -5.92 versus +17.50 pg/mL (P=0.001); side effects were less frequent on enclomiphene (odds ratio 0.18, 95% CI 0.07 to 0.44) [8]. That study was retrospective, not randomized.
Pharmacokinetics: how long the drug stays active
Oral enclomiphene is absorbed rapidly, with a peak 2 to 3 hours after a dose and a half-life of about 10 hours [1]. Its main metabolite is 4-hydroxy enclomiphene, and excretion is mainly in the feces [1]. There is no FDA-approved dose because there is no approved product [1]. A phase 2 dose-finding trial studied 6.25, 12.5 and 25 mg once daily for 6 weeks [9]. Phase 3 protocols used 12.5 mg once daily, with up-titration to 25 mg daily at week 6 if morning testosterone stayed below 300 ng/dL [4].What are the safety concerns?
FDA listed cardiac and thromboembolic events, including pulmonary embolism and deep vein thrombosis in a safety study, raised estradiol, raised PSA, raised hematocrit and no long-term safety data [1]. EMA noted a slightly higher rate of cardiac and thromboembolic events than with testosterone; cataracts were seen in nonclinical studies and EMA wanted ocular monitoring [1]. In phase 3 and EMA data, the most frequent side effects were headache, hot flushes, nausea and muscle spasm; reasons for stopping included blurred vision, muscle spasm, headache and aggression [5]. In ZA-302, headache was 7 of 112 on 12.5 mg versus 1 of 47 on placebo; nausea was 4 of 112 versus 3 of 47 [5].The clomiphene label warns about visual symptoms such as blurring, scotomata and flashes, which are usually reversible but may rarely persist [7]. That warning is for the mixture, not proven for enclomiphene alone.
Regulatory status and compounding
Enclomiphene is not FDA-approved. FDA's June 8, 2022 advisory-committee presentation says no FDA-approved drugs contain enclomiphene as an active ingredient [1]. In 2015, FDA sent the sponsor a complete response letter for Androxal, and no FDA approval has followed [1]. EMA's CHMP adopted a negative opinion on 25 January 2018 recommending refusal of marketing authorisation for EnCyzix for hypogonadotropic hypogonadism in overweight men with BMI at least 25, based on 4 main studies in 588 patients [2].Enclomiphene is not on FDA's final list of bulk substances that can be used in compounding; it is in Category 1, bulk drug substances under evaluation, in a list updated May 14, 2026 [10]. FDA says it does not intend to take action against a compounder using a Category 1 substance if the conditions in its guidance are met; that is enforcement discretion, not approval and not a finding that the product is safe or effective [10]. At the June 8, 2022 advisory committee, FDA proposed that enclomiphene citrate not be added to the allowed bulk substances list; the vote was Yes 4, No 8, Abstain 0 [11].
Where the evidence stands
The BSSM 2026 position statement calls enclomiphene promising for men with secondary hypogonadism who want to preserve fertility, but notes the evidence is from small short-term studies and does not recommend routine use outside specialist settings [6].Frequently asked questions
