Does Finasteride Reach the Brain?
The Propecia label states that finasteride crosses the blood-brain barrier [4]. It is a competitive inhibitor selective for Type II 5-alpha-reductase, roughly 100-fold over Type I, and it has no affinity for the androgen receptor [4]. One 1 mg tablet suppresses serum DHT by 65% within 24 hours, while mean testosterone and estradiol rise by about 15% and stay in the normal range [4].
The reason the brain comes up at all is the enzyme's other substrates. 5-alpha-reductase converts progesterone and testosterone into 5-alpha-reduced neurosteroids, and a 2011 review describes converging evidence that these steroids participate in brain neurotransmission and behaviour [9]. In one small study, researchers compared paired plasma and cerebrospinal fluid samples from men who had taken finasteride for hair loss and stopped against healthy controls. In CSF, progesterone and its metabolites dihydroprogesterone and tetrahydroprogesterone were lower, pregnenolone was higher, DHT was lower and testosterone was higher [10]. The same paper reports anxiety and depressive symptoms; its measured outcomes were neuroactive steroid levels in plasma and CSF [10].
What the Label Says
Neither Propecia nor Proscar carries a boxed warning. Depression and suicidal ideation appear in the Postmarketing Experience section (6.2), not as a numbered Warnings and Precautions subsection [4][6]. The labels state that these reports are voluntary, come from a population of uncertain size, and that a causal relationship cannot reliably be established [4][6].
The history matters for anyone reading the current wording. The June 2021 Propecia label listed postmarketing 'Nervous System/Psychiatric: depression'. On May 31, 2022 the FDA sent a Safety Labeling Change order under section 505(o)(4) of the FD and C Act, citing new safety information on 'the risk of suicidal ideation and behavior, additional risk of sexual dysfunction and psychoneurocognitive adverse events'. Supplement S-030, approved 08/01/2022, changed the label wording to 'depression, suicidal ideation and behavior' [5].
Did Any Trial Measure Thinking or Memory?
Not the hair-loss trials. The two pivotal Propecia studies were 1-year, placebo-controlled trials in 1,553 men aged 18 to 41, with 1,215 continuing into blinded second-year extensions; efficacy was judged by scalp hair counts, patient and investigator assessments, and expert photograph review [1]. Hair count in a 1-inch circle of vertex scalp (baseline 876 hairs) was 107 hairs higher than placebo at 1 year and 138 higher at 2 years (P<0.001), while the placebo group kept losing hair [1]. Cognition was not among the listed endpoints [1].
What those trials did record were sexual adverse events: drug-related decreased libido 1.8% versus 1.3%, erectile dysfunction 1.3% versus 0.7%, and ejaculation disorder 1.2% versus 0.7%, with 3.8% (36 of 945) reporting one or more of these versus 2.1% (20 of 934) on placebo [4].
A 2015 review looked at how finasteride hair-loss trials reported safety. Of 34 trials, none had adequate safety reporting, 76% evaluated safety for 1 year or less, and funnel plots were asymmetric in a direction suggesting under-detection of sexual adverse effects [8].
Mood Data from Large Cohorts
The largest human datasets concern depression and self-harm, not cognition.
In Ontario, 93,197 men aged 66 or older who started a 5-alpha reductase inhibitor (mostly for BPH) were matched to unexposed men. Suicide was not increased (HR 0.88, 95% CI 0.53 to 1.45). Self-harm was higher in the first 18 months only (HR 1.88, 95% CI 1.34 to 2.64). Depression was higher in the first 18 months (HR 1.94, 95% CI 1.73 to 2.16) and stayed modestly higher afterward (HR 1.22, 95% CI 1.08 to 1.37), equal to about 17 additional self-harm and 237 additional depression cases per 100,000 patient-years [7].
The Swedish register study followed 2,236,876 men aged 50 to 90, of whom 70,645 started finasteride (median age at treatment start 73). Depression was more common with finasteride (HR 1.61, 95% CI 1.48 to 1.75) and the authors reported the association was constant over time; suicide was not associated (HR 1.22, 95% CI 0.99 to 1.49) [3].
Both were observational, and both drew on older men (median age 75 in Ontario; 73 at treatment start in Sweden), unlike the younger men who typically take 1 mg for hair loss. Neither measured memory, attention or processing speed [7][3].
The Surveys That Raised the Question
These two small studies recruited men who already had persistent symptoms.
Irwig's 2012 study interviewed 61 former finasteride users with persistent sexual side effects and 29 men with hair loss who had never used the drug. Depressive symptoms (BDI-II score of 14 or more) were present in 75% (46 of 61) versus 10% (3 of 29), P<0.0001. The design was cross-sectional and self-selected, so it cannot show how common this is or prove cause [11].
A 2015 internet survey of 131 generally healthy men (mean age 24) who reported symptoms at least 3 months after stopping finasteride found that adverse effects persisted across every domain asked about, including cognitive and psychological symptoms [2]. The men were contacted because they reported symptoms, so the survey cannot show how often cognitive symptoms occur or whether finasteride caused them.
The steroid study described earlier adds a biological finding rather than a cognitive one: altered neuroactive steroid levels in CSF and plasma, including lower dihydroprogesterone and tetrahydroprogesterone, in former users who reported depressive symptoms [10].
Does Finasteride Affect Dementia Risk?
One large dataset touches this. In the Swedish cohort, finasteride was associated with all-cause dementia, but the association faded over time, and the authors said this may partly reflect more dementia detection in men already being treated for BPH [3]. The outcome measured was a recorded dementia diagnosis, not a cognitive test score.
Does the Dose Matter?
Propecia is one 1 mg tablet daily, with or without food [4]. Both the Propecia and Proscar labels list depression and suicidal ideation in the postmarketing section [4][6], and the 2022 labeling change was made to the Propecia (1 mg) label [5]. The mood signals in the two large cohorts come from older men, so they may not apply directly to younger men taking 1 mg for hair loss [7][3].
What the Evidence Does Not Answer
None of the studies described above used validated neurocognitive tests. The pivotal trials measured hair and sexual outcomes [1][4], the cohorts measured recorded diagnoses [7][3], the survey recorded self-reported symptoms [2], and the CSF study measured neuroactive steroid levels, not memory or processing speed [10].
