Exercise While on GLP-1: What to Do, What to Avoid, and How to Get the Most From Both
At a glance
- Exercise in the labeling / Wegovy and Foundayo are each indicated with a reduced calorie diet and increased physical activity [1][2]
- Nausea at the top dose / 44% on Wegovy 2.4 mg vs 16% placebo; 35% on Foundayo 17.2 mg vs 10% [1][2]
- Fatigue / 11% vs 5% on Wegovy; 9% vs 4% on Foundayo [1][2]
- Resting heart rate / Mean increase of 1 to 4 bpm on Wegovy; Foundayo 4 to 5 bpm vs 0.5 bpm on placebo [1][2]
- Dehydration / Acute kidney injury due to volume depletion is a labeled warning [1][2]
- Low blood sugar / Risk rises with insulin or a sulfonylurea [1][2]
Does either label say how to exercise?
Both weight-management labels place activity inside the approved use. The Wegovy injection label states that it is indicated in combination with a reduced calorie diet and increased physical activity to reduce excess body weight and maintain weight reduction long term in adults with obesity, or overweight with at least one weight-related condition [1]. The Foundayo (orforglipron) label uses the same wording [2].
The indication wording in both labels does not name an exercise type, a weekly minutes target or a training intensity. In STEP 1, 1,961 adults took semaglutide 2.4 mg weekly plus lifestyle intervention for 68 weeks and lost a mean 14.9% of body weight versus 2.4% with placebo [3]. The published result does not separate the effect of exercise from the rest of that program.
What makes training harder in the early weeks
Gastrointestinal effects are the common ones and they are dose-related. On Wegovy 2.4 mg, nausea occurred in 44% versus 16% on placebo, diarrhea in 30% versus 16%, vomiting in 24% versus 6% and constipation in 24% versus 11% [1]. On Foundayo 17.2 mg, nausea was 35% versus 10%, diarrhea 25% versus 11%, vomiting 24% versus 4% and constipation 24% versus 9% [2]. Rates from different trials are not directly comparable.
The Foundayo label notes that nausea, vomiting and diarrhea were more frequent during dose escalation and decreased over time [2]. Fatigue was reported by 11% on Wegovy 2.4 mg versus 5% on placebo, and dizziness by 8% versus 4% [1]; Foundayo listed fatigue at 9% versus 4% [2].
Hydration, kidneys and heart rate
The Wegovy label lists acute kidney injury due to volume depletion as a warning and says to monitor renal function in patients reporting reactions that could lead to volume depletion [1]. The Foundayo label says most reported cases followed gastrointestinal reactions causing dehydration such as nausea, vomiting or diarrhea, and advises monitoring during initiation and dose escalation [2].
Heart rate is also labeled. The Wegovy label reports mean resting heart rate increases of 1 to 4 bpm and says to monitor heart rate at regular intervals [1]; Foundayo reports 4 to 5 bpm versus 0.5 bpm on placebo [2].
Blood sugar around a workout
Wegovy lowers blood glucose and can cause hypoglycemia, with a higher risk when it is used with insulin or an insulin secretagogue such as a sulfonylurea; the label says to consider reducing those doses when starting and to monitor blood glucose before and during treatment in patients with diabetes [1]. In the Foundayo type 2 diabetes trial, glucose below 54 mg/dL was reported in 2% on orforglipron versus 0.2% on placebo, and 7% of those taking it with a sulfonylurea versus 0.5% without [2].
