Are GLP-1 Weight Loss Drugs HSA and FSA Eligible? Wegovy, Zepbound, Ozempic, and Mounjaro Costs Explained

At a glance

  • HSA and FSA eligibility / IRS Publication 502 includes prescribed medicines and drugs among medical expenses; the FDA labels for these four products do not address HSA or FSA eligibility [1][2][3][4][9]
  • Prescription status / All four are prescription-only; none is sold over the counter [1][2][3][4]
  • Approved for weight management / Wegovy (semaglutide) and Zepbound (tirzepatide) [1][3]
  • Approved for type 2 diabetes / Ozempic (semaglutide) and Mounjaro (tirzepatide) [2][4]
  • Mounjaro and Zepbound / Both are tirzepatide, labeled a dual GIP and GLP-1 receptor agonist [3][4]
  • Compounded semaglutide and tirzepatide / Not FDA approved; not reviewed for safety, effectiveness or quality [5]

What the IRS and the drug labels say

HSA and FSA spending is measured against the IRS definition of medical expenses. IRS Publication 502 says you can include in medical expenses amounts you pay for prescribed medicines and drugs, and it defines a prescribed drug as one that requires a prescription by a doctor for its use by an individual [9]. Its weight-loss section says you can include amounts you pay to lose weight if it is a treatment for a specific disease [9].

On the drug side, every current US GLP-1 product is a prescription injection or tablet, and none is sold over the counter [1][6]. The FDA labeling for Wegovy, Zepbound, Ozempic and Mounjaro describes each drug, its approved uses and its safety information. It does not address HSA or FSA eligibility, and it does not provide list prices, savings-program figures or patient-assistance details [1][2][3][4].

Which of the four treats what

Wegovy (semaglutide). The injection is labeled, with a reduced-calorie diet and more physical activity, to reduce excess body weight and maintain weight reduction long term in adults and children aged 12 and older with obesity, and in adults with overweight plus at least one weight-related condition. It is also labeled to reduce major cardiovascular events in adults with established cardiovascular disease and either obesity or overweight, and to treat MASH with moderate to advanced liver fibrosis in adults [1]. Wegovy tablets are labeled for weight reduction and cardiovascular risk reduction in adults [1].

Zepbound (tirzepatide). Labeled for weight management and for sleep apnea [3].

Ozempic (semaglutide). Labeled for type 2 diabetes, for cardiovascular risk reduction in type 2 diabetes with established cardiovascular disease, and for kidney outcomes in type 2 diabetes with chronic kidney disease [2].

Mounjaro (tirzepatide). Labeled for type 2 diabetes [4].

Among these four products, Wegovy and Zepbound carry FDA-approved weight-management indications, while Ozempic and Mounjaro carry FDA-approved type 2 diabetes indications [1][2][3][4].

Semaglutide and tirzepatide are different drugs

Ozempic and Wegovy both contain semaglutide, a GLP-1 analogue [1][2]. Mounjaro and Zepbound both contain tirzepatide, which the FDA label describes as a GIP receptor and GLP-1 receptor agonist: a dual agonist rather than a pure GLP-1 drug, although it is commonly grouped with GLP-1 drugs [3][4].

Compounded semaglutide and tirzepatide

Compounded drugs are not FDA approved, and the agency does not review them for safety, effectiveness or quality before they are marketed [5].
