What a plateau actually means in the trial data

A post-hoc analysis of two tirzepatide trials defined a plateau as weight change of under 5% across a 12-week interval, and across every 12-week interval after that [1]. By that definition, someone still losing a few kilograms every three months would be counted as plateaued.

The analysis included 1,438 participants from SURMOUNT-1 and 259 from SURMOUNT-4 who stayed on treatment and lost at least 5% of body weight [1]. In SURMOUNT-1, median time to plateau was 24.3 weeks for people who started with overweight, 26.0 weeks for class I obesity and 36.1 weeks for class II or III obesity; by week 72, 87.6% to 90.2% had reached a plateau, depending on BMI class [1]. Higher doses (10 or 15 mg), younger age and female sex were each associated with a later plateau, and the SURMOUNT-4 results were similar [1].

So a plateau is the usual end state of treatment in these data, not an anomaly. What varies is when it arrives.

Dose and weight loss

Weight loss on these drugs depends heavily on dose. SURMOUNT-1 assigned 2,539 adults with obesity or overweight and no diabetes to tirzepatide 5, 10 or 15 mg once weekly or placebo for 72 weeks: mean weight change was -15.0%, -19.5% and -20.9% versus -3.1% [3]. STEP 1 assigned 1,961 similar adults to semaglutide 2.4 mg weekly or placebo for 68 weeks: -14.9% versus -2.4% [4].

Both weight-management labels build in a slow climb rather than a fixed dose. Wegovy injection starts at 0.25 mg once weekly for 4 weeks and increases every 4 weeks to a usual maintenance dose of 2.4 mg; Wegovy tablets start at 1.5 mg daily for 30 days and rise every 30 days to 25 mg [7]. Foundayo starts at 0.8 mg daily for at least 30 days, then 2.5 mg, 5.5 mg, and then steps of at least 30 days up to a maximum of 17.2 mg [8].

STEP 1 used semaglutide 2.4 mg, so a stall at an earlier step of the escalation schedule is below the dose that produced the trial result [7][4].

Tirzepatide versus semaglutide

SURMOUNT-5 randomized 751 adults with obesity and no diabetes to the maximum tolerated dose of tirzepatide (10 or 15 mg) or semaglutide (1.7 or 2.4 mg) for 72 weeks, open-label: weight change was -20.2% versus -13.7%, and waist circumference fell 18.4 cm versus 13.0 cm [5]. That trial measured overall weight loss on each drug; it did not test switching drugs after a plateau.

Tirzepatide is labelled as a GIP and GLP-1 receptor agonist, a dual agonist rather than a pure GLP-1 drug [9]. Whether the added GIP activity explains the gap is not established by these trials [9][10]. In SURPASS-2, which compared the two in 1,879 adults with type 2 diabetes over 40 weeks, tirzepatide 5, 10 and 15 mg lowered HbA1c by 2.01, 2.24 and 2.30 points versus 1.86 with semaglutide 1 mg, with 1.9, 3.6 and 5.5 kg more weight loss [10].

What happens when the drug is stopped or swapped

Two trials speak directly to the period after weight loss has flattened.

STEP 4 took 803 adults who had completed a 20-week semaglutide run-in and reached the 2.4 mg weekly maintenance dose, then randomized them to continue semaglutide or switch to placebo for 48 weeks: the continued group lost a further 7.9% of body weight while the placebo group gained 6.9%, a difference of 14.8 points [6].

ATTAIN-MAINTAIN randomized people previously treated with tirzepatide (205 participants) or semaglutide (171) in SURMOUNT-5 to once-daily oral orforglipron or placebo for 52 weeks. Among those who had reached a body weight plateau, the prior-tirzepatide group maintained a model-based estimate of 74.7% of its weight reduction on orforglipron versus 49.2% on placebo; the prior-semaglutide group maintained 79.3% versus 37.6% [2]. The trial had no arm continuing the injectable, so it does not compare orforglipron with staying on the original drug [2].

The Wegovy and Foundayo labels state that concomitant use with another GLP-1 receptor agonist is not recommended [7][8].

Less frequent dosing: one small case series

A 2026 retrospective case series described 30 adults who had plateaued on weekly semaglutide or tirzepatide and moved to reduced-frequency dosing, usually every other week, at their existing dose. Over a mean 36.3 weeks on that schedule, average weight went from 74.1 kg at plateau to 72.4 kg, total and truncal fat declined, and skeletal muscle mass stabilized; the metabolic improvements gained on weekly dosing were maintained [11]. Everyone in that series had already lost weight and there was no comparison group, so it cannot show that less frequent dosing works as well as weekly dosing. The labeled doses are the ones that produced the STEP 1 and SURMOUNT-1 results [4][3].

Why the labels escalate slowly

The Foundayo label says to follow its starting dose and escalation to reduce the risk of GI adverse reactions, and that nausea, vomiting and diarrhea were more common during escalation and decreased over time [8]. In trials of Wegovy 2.4 mg weekly injection, nausea occurred in 44% versus 16% on placebo and vomiting in 24% versus 6%; 6.8% of Wegovy-treated participants stopped treatment because of adverse reactions versus 3.2% on placebo [7]. At the Foundayo maximum 17.2 mg dose, nausea was 35% versus 10% and vomiting 24% versus 4%; 10% at that dose discontinued for adverse reactions versus 3% on placebo [8]. Rates from separate trials are not directly comparable [7].

Reading a plateau against the trial definitions

The tirzepatide analysis used a 12-week window and a 5% threshold, so a fortnight of no movement is not the same event [1]. The trial results come from specific maintenance doses: 2.4 mg weekly for Wegovy injection, 25 mg daily for Wegovy tablets, up to 17.2 mg for Foundayo, and 10 or 15 mg in the tirzepatide comparison trial [7][8][5]. The Wegovy and Foundayo labels specify use alongside a reduced-calorie diet and increased physical activity [7][8].

Frequently asked questions

When do people usually hit a GLP-1 plateau?
In the tirzepatide analysis, a plateau was weight change under 5% across a 12-week interval, and the median time to reach it was 24.3 to 36.1 weeks depending on starting BMI; by week 72, 87.6% to 90.2% of adherent SURMOUNT-1 participants who lost at least 5% had reached one [1].
Is it normal to stop losing weight on semaglutide?
The plateau analysis covered tirzepatide, where most adherent participants plateaued by week 72 [1]. For semaglutide, STEP 1 reported a mean weight change of -14.9% after 68 weeks with 2.4 mg versus -2.4% with placebo in 1,961 adults [4], and in STEP 4, participants who continued semaglutide after a 20-week run-in lost a further 7.9% over 48 weeks [6].
Does tirzepatide work better than semaglutide if I plateau?
Tirzepatide produced greater average weight loss than semaglutide over 72 weeks in people with obesity without diabetes: -20.2% versus -13.7% in 751 adults [5]. That trial did not test whether switching to tirzepatide breaks an existing plateau. Tirzepatide is a GIP and GLP-1 receptor agonist rather than a pure GLP-1 agonist [9], though whether the extra GIP activity causes the difference is not established [9][10].
Does less frequent dosing keep weight off after a plateau?
One retrospective case series followed 30 adults who had plateaued on weekly semaglutide or tirzepatide and moved to reduced-frequency dosing, usually every other week at the same dose; over a mean 36.3 weeks weight went from 74.1 kg to 72.4 kg and muscle mass stayed stable [11]. With no control group, it cannot show that less frequent dosing matches weekly dosing.
What happens if I stop at a plateau?
In STEP 4, 803 adults who completed a 20-week semaglutide run-in and reached the 2.4 mg weekly maintenance dose were randomized to continue semaglutide or switch to placebo for 48 weeks: -7.9% versus +6.9% [6]. In ATTAIN-MAINTAIN, people previously treated with tirzepatide who had reached a plateau kept an estimated 74.7% of their weight reduction on orforglipron versus 49.2% on placebo [2].
Is a flat scale for two weeks a plateau?
Not by the standard used in the tirzepatide analysis, which required weight change under 5% across a full 12-week interval, and across each 12-week interval after that [1].
