Is anything approved for post-bariatric weight regain?
No. Wegovy injection and tablets, Saxenda and generic liraglutide for weight, Zepbound and Foundayo carry weight-management indications [1][3][4][2]. Wegovy covers adults with obesity, or overweight plus at least one weight-related comorbid condition [1]. Foundayo covers the same population, and its label says use with another GLP-1 receptor agonist is not recommended [2].
Ozempic and Rybelsus are indicated for type 2 diabetes; Victoza, Trulicity, Byetta, Mounjaro, Soliqua and Xultophy also carry diabetes indications rather than weight-management indications [8][9][10][11][12][13][14][15]. Each product's label criteria are the same whether or not a person has had bariatric surgery: obesity or overweight with a weight-related condition for the weight-management products, and type 2 diabetes for the diabetes products [1][2][3][4][8][9][12][15].
What the obesity trials measured
STEP 1 enrolled 1,961 adults with a BMI of 30 or higher, or 27 or higher with a weight-related condition, and no diabetes. Over 68 weeks, weight changed -14.9% with semaglutide 2.4 mg weekly versus -2.4% with placebo [5]. SURMOUNT-1 enrolled 2,539 adults without diabetes for 72 weeks: -15.0%, -19.5% and -20.9% at 5, 10 and 15 mg of tirzepatide versus -3.1% with placebo [6]. SURMOUNT-5 randomized 751 adults with obesity and no diabetes, open-label, to a maximum tolerated dose of either drug for 72 weeks: -20.2% with tirzepatide versus -13.7% with semaglutide [16].
Stopping treatment
STEP 4 randomized 803 adults who reached semaglutide 2.4 mg during a 20-week run-in to continue the drug or switch to placebo for 48 weeks: weight fell another 7.9% on semaglutide and rose 6.9% on placebo [7].
What the labels say to watch for
Wegovy, Saxenda, Zepbound and Foundayo carry boxed warnings about the possible risk of thyroid C-cell tumors and are contraindicated with a personal or family history of medullary thyroid carcinoma or MEN 2. The Wegovy and Zepbound warnings describe rodent tumors; the Foundayo label says orforglipron was not pharmacologically active in rats or mice and did not produce tumors in rodents, while the human relevance of GLP-1-receptor-dependent rodent tumors remains undetermined [1][2][3][4].
The Wegovy label lists acute pancreatitis, with instructions to discontinue if it is suspected; acute gallbladder disease; acute kidney injury from volume depletion; hypoglycemia with insulin or a sulfonylurea; and pulmonary aspiration during general anesthesia or deep sedation, and says patients should tell their surgical team about planned procedures. It does not recommend Wegovy in people with severe gastroparesis [1]. In the weight-management trials, nausea was 44% versus 16% and vomiting 24% versus 6%, and 6.8% versus 3.2% stopped treatment for adverse reactions [1].
