Zepbound payment-route and quote worksheet

Write down the exact Zepbound presentation first, pick the route that matches your coverage, and compare written quotes line by line, including the refill price and any care fees. A Medicare section lists the Bridge rules from Medicare.gov, and a final section records a denial so the next step is clear.

  • Commercial plan covers Zepbound: the pharmacy's covered price and the current commercial savings terms
  • Commercial plan excludes Zepbound: noncovered savings terms and a separate self-pay quote for the same presentation
  • No insurance: the current self-pay offer and an itemized pharmacy quote
  • Medicare: whether the claim belongs under Part D or the Medicare GLP-1 Bridge
  • Medicaid, VA or TRICARE: the program's own benefit and authorization process
  • Lilly Cares: confirm Zepbound is on its current list before sending any income records

Medicare GLP-1 Bridge rules (Medicare.gov)

  • The copayment is $50 for a one-month supply, either 28 or 30 days depending on the drug.[1]
  • For Zepbound the Bridge covers the KwikPen; single-dose vials and pens are not covered.[1]
  • The Bridge copayment does not count toward your Part D deductible or out-of-pocket limit, and Extra Help cannot lower it.[1]
  • An approved Bridge prior authorization covers refills and dose changes through December 31, 2027, unless you switch GLP-1s.[1]
  • Buying with a discount card or TrumpRx price instead of your Medicare plan does not count toward your Medicare deductible or out-of-pocket maximum.[1]
