What the two current labels say

  • Wegovy injection starts at 0.25 mg once weekly for weeks 1 to 4, then 0.5 mg (weeks 5 to 8), 1 mg (weeks 9 to 12) and 1.7 mg (weeks 13 to 16), reaching the maintenance dose from week 17.[1]
  • Saxenda (liraglutide 3 mg) starts at 0.6 mg daily for one week, then 1.2, 1.8 and 2.4 mg in weeks 2 to 4, and 3 mg daily from week 5.[2]
  • Saxenda: if more than 3 days have passed since the last dose, restart at 0.6 mg daily and step up again.[2]
  • Neither medicine is meant to be combined with another GLP-1 receptor agonist: both labels say so.[1]
  • Saxenda uses a multi-dose pen; pen needles are not included, and the pen in use is thrown away after 30 days.[2]
  • Wegovy injection comes in single-dose pens at every strength.[1]

Call your healthcare provider right away

  • Severe pain in your stomach area that will not go away, with or without nausea or vomiting: both Medication Guides say to stop the medicine and call right away.[1]

To settle with the prescriber before the switch

  1. On what date should my first dose of the new medicine be, given my last dose of the old one?
  2. Do I start the new medicine at its lowest label dose?
  3. Which pen needles do I need for liraglutide, and how is the dose dialed?
  4. Does my plan cover the new medicine, and is prior authorization needed?
