From Blue Cross NC's July 2026 policy

  • Coverage under this policy requires anti-obesity medication benefits in the member's plan.[1]
  • When the member's formulary excludes Wegovy, non-formulary exception criteria apply as well.[1]
  • A first approval lasts 180 days; a continuation lasts 365 days.[1]
  • For weight management, continuation needs at least 5% weight loss from the pre-treatment baseline, kept off.[1]
  • The 0.25 mg, 0.5 mg and 1 mg pens are limited to 4 pens per 28 days.[1]
  • A criterion about previous weight-loss medication trials was removed in April 2023.[1]

North Carolina external review

  • It is free, and it is for denials of medically necessary treatment.[2]
  • File within 120 days of the insurer's final appeal decision.[2]
  • Smart NC says within 10 business days whether a standard request is complete and accepted; the decision follows within 45 days of the request.[2]
  • Self-funded employer plans are outside the state process.[2]
  • The reviewer's decision binds you and the health plan.[2]

If you have Medicare drug coverage

  • Medicare's GLP-1 Bridge has covered Wegovy injection or tablets for eligible people since July 1, 2026, at $50 for a one-month supply.[3]
