Wegovy kidney tracker: eGFR, urine albumin and fluid losses

Record your starting eGFR, urine albumin and kidney-affecting medicines, then add each result with the Wegovy dose and any vomiting or diarrhea before the test, so a single low reading can be judged against the trend and against dehydration.

Kidney results over time

A low reading drawn after a day of vomiting or poor drinking is worth repeating once you are rehydrated.

What SELECT and the label report

  • SELECT, semaglutide 2.4 mg: eGFR declined 0.75 mL/min/1.73 m² less than on placebo by 104 weeks.[1]
  • SELECT: urine albumin rose less on semaglutide, a net difference of 10.7% at 104 weeks.[1]
  • SELECT: the main kidney composite occurred in 1.8% on semaglutide versus 2.2% on placebo (hazard ratio 0.78).[1]
  • SELECT found no excess of acute kidney failure on semaglutide, whatever the starting eGFR.[1]
  • The Wegovy label reports acute kidney injury after dehydration from stomach side effects, sometimes needing dialysis, and says to monitor kidney function when that happens.[2]

Call your healthcare provider for

  • Vomiting or diarrhea that stops you keeping fluids down, little or dark urine, or dizziness on standing: dehydration can injure the kidneys.[2]

Questions about my kidney numbers

  1. Is this eGFR change bigger than the normal test-to-test variation for me?
  2. Should we repeat the test after I am well hydrated before deciding anything?
  3. Should my urine albumin be checked as well as eGFR, and how often?
  4. Do any of my medicines need adjusting while I am losing weight or having stomach side effects?
  5. Given my diabetes status, which trial results apply to me more closely, SELECT or FLOW?
