Gabapentin with Ozempic: kidney, timing and drowsiness record

Keep your gabapentin dose, your latest kidney result and your antacid use on one page, and log pain or seizure control, drowsiness and fluid losses at each Ozempic dose step, so a prescriber or pharmacist can see whether anything needs to change.

From the Neurontin label and the gastric-emptying study

  • Gabapentin absorption falls as the dose rises: bioavailability is about 60% at 900 mg/day and 33% at 3,600 mg/day.[1]
  • Food has only a slight effect on gabapentin absorption (14% higher AUC and Cmax).[1]
  • A magnesium and aluminum antacid (Maalox) lowered gabapentin bioavailability by about 20%; the label advises taking gabapentin at least 2 hours after it.[1]
  • Gabapentin leaves the body unchanged through the kidneys; its half-life is 5 to 7 hours.[1]
  • Semaglutide 1 mg lowered first-hour gastric emptying by 27%, with no significant difference over 5 hours; gabapentin itself was not tested.[2]

Get help or call right away

  • Breathing problems, especially if you also take an opioid or already have breathing problems: short of breath, breathing slower than normal, very tired, confused, dizzy or headache (Neurontin guide: call your provider or get medical help right away).[1]
  • Severe stomach pain that will not go away, with or without vomiting (Ozempic guide: stop Ozempic and call right away).[3]
  • Nausea, vomiting or diarrhea that does not go away (Ozempic guide: tell your provider right away).[3]
  • Do not stop gabapentin without first talking to your healthcare provider.[1]

For the prescriber or pharmacist

  1. Does my gabapentin dose still match my latest kidney result?
  2. If I have vomiting or diarrhea for more than a day, should my gabapentin dose change?
  3. Should I take gabapentin at the same times relative to meals now that I am on Ozempic?
  4. Which of my other medicines add to drowsiness?
  5. If pain control slips after an Ozempic dose increase, would pregabalin be worth discussing?
