EGRIFTA with levothyroxine: thyroid results and what changed

Record why you take levothyroxine, your pituitary history, and each TSH or free T4 result next to anything that changed at the time, so a new result can be read against the levothyroxine label's own rules before anyone blames EGRIFTA.

What the two labels say

  • EGRIFTA trials showed no clinically significant change in TSH or other pituitary hormones.[1]
  • EGRIFTA is contraindicated with disruption of the hypothalamic-pituitary axis from hypophysectomy, hypopituitarism, pituitary tumor or surgery, head irradiation or head trauma.[1]
  • Primary hypothyroidism: TSH 6 to 8 weeks after any levothyroxine dose change, then every 6 to 12 months while stable.[2]
  • Secondary or tertiary hypothyroidism: TSH is not a reliable guide to the levothyroxine dose.[2]
  • Keep levothyroxine at least 4 hours apart from medicines that interfere with its absorption.[2]
  • Stop biotin supplements at least 2 days before a TSH or T4 test.[2]

For the prescriber or endocrinologist

  1. Which kind of hypothyroidism do I have, and should we follow TSH or free T4?
  2. Does anything in my pituitary history matter for EGRIFTA?
  3. Is a thyroid test due anyway, regardless of EGRIFTA?
  4. If a result changes, what else should we rule out before changing my dose?
  5. Should I keep my levothyroxine routine exactly as it is?
