Wegovy with estradiol HRT: route, morning order and warning-sign record

Write down how each hormone is taken and, if you use Wegovy tablets, the order you take things in the morning. A dose-change record and the warning signs from both labels go to the prescriber who manages your HRT.

From the two labels and the WHI trial

  • Wegovy tablets go on an empty stomach with up to 4 ounces of water; wait at least 30 minutes before food, drinks or other oral medicines.[1]
  • With either Wegovy form, the label asks for the effects of oral medicines to be monitored.[1]
  • In the WHI trial of conjugated estrogens with medroxyprogesterone (not estradiol), venous clots occurred at 3.5 per 1000 person-years on hormones and 1.7 on placebo.[2]
  • Talk with your prescriber regularly, for example every 3 to 6 months, about the estradiol dose and whether you still need it.[3]
  • Where feasible, estrogen is stopped at least 4 to 6 weeks before surgery that carries a higher clot risk, or during long immobilization.[3]

Call your healthcare provider right away

  • Chest pain, shortness of breath, pains in your legs, changes in speech or vision, severe headaches, dizziness and faintness, breast lumps, unusual vaginal bleeding, or vomiting (the estradiol label's warning signs).[3]
  • Severe stomach pain that will not go away, with or without nausea or vomiting: stop Wegovy and call right away.[1]

For the HRT prescriber

  1. Given my weight and clot history, should my estrogen be by mouth or through the skin?
  2. If I move to Wegovy tablets, where in the morning should my hormone tablet go?
  3. Which symptoms or levels will we review after I start Wegovy, and when?
  4. If I lose a lot of weight, will we review the HRT dose?
  5. If I vomit soon after a hormone tablet, should I take another?
