Trazodone and Zepbound bedtime and morning record
Log each night's trazodone dose, what you ate beforehand, how long sleep took and how you felt on standing the next morning, next to your Zepbound dose and injection day. A change after a Zepbound dose increase is then easy to show the prescriber, and the label warnings that need urgent action are on the page.

From the two labels
- Take trazodone shortly after a meal or light snack.[1]
- Trazodone can cause sleepiness; do not drive or use hazardous machinery until you know how it affects you, and avoid alcohol or other sedating medicines unless your provider agrees.[1]
- Trazodone can cause low blood pressure on standing and fainting; a blood pressure medicine dose may need to come down.[1]
- Trazodone should be avoided with known QT prolongation or with CYP3A4-blocking drugs such as itraconazole, clarithromycin or voriconazole.[1]
- Serotonin syndrome risk rises when trazodone is combined with an SSRI, SNRI or triptan, but it can also occur with trazodone alone.[1]
- Low blood pressure was reported in 1.6% of people on Zepbound and 0.1% on placebo in its weight trials.[2]
Do not wait
- An erection lasting more than 4 hours, painful or not: stop trazodone and get emergency medical care.[1]
- New or worse depression or anxiety, agitation, panic attacks, trouble sleeping, or thoughts of suicide: call a healthcare provider right away.[1]
- Severe stomach pain that will not go away, with or without nausea or vomiting: stop Zepbound and call your healthcare provider right away.[2]
For the prescriber or pharmacist
- Sleep has taken longer since my Zepbound dose went up: should anything change, or should we wait?
- Should I take trazodone at the same time and with the same snack every night?
- Do my blood pressure medicines need checking now that I am losing weight?
- Do any of my other medicines raise trazodone levels or affect the QT interval?
