Before anyone proposes an MK-677 regimen to a young adult: questions to ask

Write down what was proposed and by whom, then check whether the person proposing it can show each input a real dosing recommendation needs. It is a record for a conversation with a clinician; it contains no dose and does not create one.

Can they show each input a real dosing recommendation needs?

Tick only what they can document. An empty box means the number they gave you was not validated.

  • A defined indication and the outcome being targeted
  • A characterized product and formulation
  • Dose-response or exposure-response evidence in this age group
  • A safety margin measured in people like me
  • A monitoring and adjustment rule tested against outcomes

What the evidence behind the numbers looks like

  • The most directly relevant healthy young-men study enrolled nine men for 7 days per arm and measured hormones, not benefit or long-term safety.[1]
  • In men with obesity, an oral glucose tolerance test showed impaired glucose handling at 2 and 8 weeks even though fasting glucose was unchanged.[2]
  • FDA describes most ibutamoren clinical data as small, short and without clinically meaningful benefit shown.[3]
  • FDA lists a potential congestive heart failure risk in certain patients as a significant safety concern for ibutamoren.[4]

Questions for the person proposing it

  1. Which study does this number come from, and who was in that study?
  2. Was the study designed to choose a dose, or only to measure hormones?
  3. What would make you tell me to stop, and how would we detect it?
  4. How do you know the product matches the one studied?
  5. Is there an approved treatment or a registered trial for my actual goal?
