Liver tracker for tirzepatide in MASLD: enzymes, scans, weight and gallbladder symptoms

Record the liver tests and scans this page lists at baseline, 12 weeks and 24 weeks and every 6 months after that, beside your weight and dose. It keeps the label's gallbladder warning signs and the trial results that set realistic expectations.

What the trials and the label say

  • In SYNERGY-NASH, MASH resolved without worse fibrosis in 44% to 62% of people on tirzepatide, against 10% on placebo, after 52 weeks.[1]
  • In SURPASS-3 MRI, liver fat fell 8.09 points on tirzepatide 10 mg or 15 mg, against 3.38 points on insulin degludec.[2]
  • The label recommends no Mounjaro dose change for hepatic impairment.[3]
  • In placebo-controlled adult trials, gallbladder problems (gallstones, biliary colic or gallbladder removal) were reported by 0.6% on Mounjaro and 0% on placebo.[3]

Do not wait for the next visit

  • Pain in your upper stomach, fever, yellowing of your skin or eyes, or clay-colored stools (possible gallbladder problems): tell your healthcare provider right away.[3]
  • Severe stomach pain that will not go away, with or without vomiting: stop Mounjaro and call your healthcare provider right away.[3]

Questions for the visit

  1. Which test will show whether my liver is improving?
  2. Do I need a fibrosis assessment before starting?
  3. What weight change are we aiming for, and by when?
  4. Is a gallbladder ultrasound worth doing before I start?
  5. Is resmetirom or another liver-specific treatment an option for me?
