What UHC's own documents say
- Program 2026 P 1332-8, effective July 1, 2026, approves Mounjaro when chart notes confirm type 2 diabetes with A1C 6.5% or higher, fasting glucose 126 mg/dL or higher, or a 2-hour glucose test of 200 mg/dL or higher (or random glucose with symptoms).[1]
- For a diagnosis made more than 2 years ago, chart notes confirming it are enough.[1]
- Authorization is issued for 12 months.[1]
- Medications used for weight loss are typically excluded from benefit coverage.[1]
- The drug list effective September 1, 2026 lists Mounjaro in Tier 2 with PA and QL; UHC notes that Tier 2 was called preferred brands in certain documents.[2]
- Where the drug list and the plan differ, the benefit plan documents govern.[2]
Calls, submissions and decisions
| Date | Who (UHC, OptumRx, prescriber, pharmacy) | What was sent or said | Reference number | Next deadline |
|---|---|---|---|---|
Federal appeal clocks for most private plans
- File the internal appeal within 180 days of the denial notice.[3]
- Request external review in writing within 4 months of the final denial.[4]
- Standard external reviews are decided no later than 45 days after the request; expedited reviews no later than 72 hours.[4]
- The insurer is required by law to accept the external reviewer's decision.[4]
If the request is denied
- Denial letter saved, with the exact reason and the deadline it gives
- Reason matched: missing diagnosis proof, plan exclusion, quantity limit, or something else
- Lab report or chart note added for the criterion the letter names
- Prescriber asked about a peer-to-peer call with the reviewer
- Appeal sent by a method that gives proof of delivery
Questions for UHC or OptumRx
- Does my plan use the standard UHC Prescription Drug List, or its own list?
- Was this request reviewed for type 2 diabetes, or as a weight-loss prescription?
- Which record was missing, and who should send it?
- Does the approval cover every Mounjaro strength I will step up to?
- What will a fill cost on my plan before and after the deductible?
