Rybelsus switch diary: the weeks either side of a change
Keep this from your last week on the old medicine to the first weeks on the new one: what you switched from and to, why, a daily record of how you took it and how you felt, and the questions to ask before your next refill.

Why you are switching
- Nausea, vomiting or other stomach effects
- The morning fast and 30-minute wait are hard to keep
- Other morning medicines, such as levothyroxine, clash with the routine
- Not enough effect on glucose or weight
- Formulary, prior authorization or step-therapy change
- Tired of injections and moving to tablets
- My Rybelsus strength is being discontinued
Points from FDA and the label
- An FDA drug shortage record posted 06/04/2026 lists Rybelsus 3 mg, 7 mg and 14 mg tablets as To Be Discontinued, to be replaced by Ozempic (semaglutide) tablets.[1]
- Rybelsus and Ozempic tablets are not substitutable on a mg-to-mg basis, so check the strength on every new bottle.[2]
- Both tablets are taken once a day on an empty stomach in the morning with up to 4 ounces of plain water, then nothing to eat, drink or swallow for at least 30 minutes.[2]
- In PIONEER 4, oral semaglutide 14 mg lowered HbA1c by 1.2 points at week 26, against 1.1 with liraglutide injection and 0.2 with placebo.[3]
