Wegovy diarrhea: antidiarrheal dose and fluid tracker
Note each loose stool, every antidiarrheal dose and how much you drank, alongside your Wegovy dose and injection day, so you can see when a label's time limit has passed and what to tell your prescriber. The dosing limits and stop signs from each label are printed alongside.

Label limits
- Imodium A-D (adults and children 12 and over): 2 caplets after the first loose stool, then 1 after each loose stool, no more than 4 caplets in 24 hours.[1]
- Pepto-Bismol caplets (adults and children 12 and over): 2 caplets every half hour or 4 caplets every hour as needed, no more than 8 doses (16 caplets) in 24 hours, for no more than 2 days.[2]
- Lomotil, age 13 and older: starts at 2 tablets four times daily, at most 20 mg of diphenoxylate a day; stop if chronic diarrhea has not improved within 10 days at the maximum dose.[3]
- Wegovy: if a dose is not tolerated during escalation, the label suggests delaying the next increase for 4 weeks.[4]
- STEP 1 to 3: one diarrhea episode on semaglutide 2.4 mg lasted a median of 3 days.[5]
Call your prescriber
- On loperamide: diarrhea lasting more than 2 days, symptoms getting worse, or a swollen or bulging abdomen.[1]
- On bismuth subsalicylate: ringing in the ears or loss of hearing.[2]
- Signs of dehydration: fluid loss from diarrhea can lead to kidney problems.[4]
To ask your prescriber
- If loperamide is not enough, is a prescription option right for me?
- Should my next Wegovy increase be delayed?
- Could an infection, recent antibiotics or another medicine be the cause?
- Should my metformin timing or form change?
