Wegovy nausea medicine plan: what to take, when, and what to watch

Use this with your prescriber to set an anti-nausea plan for the current dose step: which medicine, the agreed dose and timing, how you will judge whether it works, and when to try stopping it. A daily log and the label cautions for ondansetron and metoclopramide sit on the same page.

Label and trial points

  • Wegovy label: if a dose is not tolerated during escalation, consider delaying escalation for 4 weeks; if 2.4 mg is not tolerated, it can be lowered to 1.7 mg.[1]
  • STEP 1 to 3: stomach side effects led to a dose reduction or pause in 12.5% on semaglutide 2.4 mg against 1.7% on placebo.[2]
  • Ondansetron label: constipation in 9% on 8 mg twice daily against under 1% on placebo, and headache in 24% against 13%.[3]
  • Ondansetron label: ECG monitoring is recommended with low potassium or magnesium, heart failure, slow heart rhythm or other QT-prolonging medicines.[3]
  • Reglan label: the risk of tardive dyskinesia rises with treatment length and total dose; for reflux, the maximum course is 12 weeks.[4]

Stop and get help

  • Severe pain in the stomach area that will not go away, with or without nausea or vomiting: stop Wegovy and call your healthcare provider right away.[1]
  • Nausea, vomiting or diarrhea that does not go away: tell your healthcare provider right away.[1]
  • Agitation, hallucinations, a racing heart, sweating, flushing or fever while on ondansetron can be serotonin syndrome: stop it and seek care.[3]

To ask before you start

  1. Is ondansetron safe with my other medicines and my heart history?
  2. Should I take it before meals or only when nausea starts?
  3. What should I use if it adds to the constipation from Wegovy?
  4. If nausea lasts beyond this dose step, do we delay the next increase or lower the dose?
