Before kidney labs on Mounjaro: creatine disclosure card

Hand this to whoever orders or reads your kidney tests. It records your creatine use alongside your recent stomach symptoms and Mounjaro dose changes, so a creatinine result can be read with both in mind, and lists the trial facts behind each.

Evidence for the person reading the result

  • Mounjaro's label: acute kidney injury has been reported, sometimes needing dialysis; it advises checking kidney function in people with side effects that could cause fluid loss, especially when starting or raising the dose.[1]
  • In a 12-week trial in adults with type 2 diabetes doing exercise training, creatine did not change kidney filtration measured by 51Cr-EDTA clearance.[2]
  • In a 12-week trial of creatine 5 g a day with exercise training in type 2 diabetes, HbA1c went from 7.4% to 6.4% on creatine.[3]
  • Across 22 trials in 721 older adults doing resistance training, creatine added 1.37 kg of lean tissue mass compared with placebo.[4]

Report these without waiting for the next lab

  • Nausea, vomiting or diarrhea that keeps going: Mounjaro's label says the fluid loss (dehydration) may cause kidney problems.[1]

When the result comes back

  1. Is this eGFR based on creatinine only, and would a cystatin C test change how you read it?
  2. Do you want me off creatine before a repeat draw, and for how many days?
  3. Could my recent vomiting or diarrhea explain part of this result?
  4. Which result do we treat as my starting point while I take creatine?
  5. Does this result change my Mounjaro dose plan?
