Question record for a tirzepatide discussion before age 18

Use this before or during an appointment where tirzepatide is raised for someone under 18. It records the diagnosis, what each proposal relies on, and the questions that show whether a plan rests on adolescent evidence or on adult extrapolation. It contains no dose, because no Zepbound dose exists for this age.

What is being discussed

Treatment goal in the family's words
Product and dose proposed, and the source cited for itadult label, adult trial, diabetes trial, adolescent obesity trial
Approved pediatric options already considered

Where the evidence stands

  • Zepbound's label states that its safety and effectiveness have not been established in pediatric patients.[1]
  • The adolescent obesity trial NCT06075667 enrolled 160 participants aged 12 to 17. Its registry record shows no posted results.[2]
  • SURPASS-PEDS studied tirzepatide in young people aged 10 to under 18 with type 2 diabetes, not obesity.[3]
  • Wegovy's label covers pediatric patients aged 12 years and older with obesity. That is a Wegovy approval, not a Zepbound dose.[4]

Questions to put to the clinician

  1. Which diagnosis and treatment goal is this proposal for?
  2. Which approved pediatric treatments were considered, and why were they set aside?
  3. Is the proposed product and dose backed by an adult label, an adult trial, a diabetes trial, or an adolescent obesity trial?
  4. Besides weight, which outcomes will show benefit: daily function, metabolic results, quality of life, growth?
  5. Which benefits and harms remain unknown while adolescent obesity results are unpublished?
  6. Who monitors treatment, how often, and when will it be reassessed or stopped?

Covered before any decision

  • The missing pediatric indication was explained in plain language
  • Growth, puberty, bone health and mood were discussed
  • We know what is expected to happen if treatment stops
  • Side effects to watch for, and who to call, are written down
  • A date to review the decision is set

Do not wait for an appointment if any of these happen

  • Belly pain that is severe or keeps going, possibly reaching through to the back, whether or not there is vomiting.[1]
  • Swollen face, lips, tongue or throat, or difficulty breathing or swallowing.[1]
  • An extra or accidental injection: call Poison Help on 1-800-222-1222.[1]
