Zepbound late-dose worksheet: hours, device and next injection
Work out a late dose on paper before you inject: hours since it was due, which device you use and how many doses it has left, and the date of your next regular injection. It keeps a running injection record and the signs that come before any timing question.
The dose that was missed
Date and time the injection was due
Date and time you noticed
Hours since it was duecount from the scheduled time, not from when you noticed
Strength (mg)
Devicesingle-dose pen, single-dose vial, multi-dose vial or KwikPen
Multi-dose vial or KwikPen only: date first used and doses already taken
Label rules for one late dose
- Up to 4 days (96 hours) after the scheduled time: take the missed dose as soon as you can.[1]
- More than 4 days: skip it and take the next dose on your regular day.[1]
- Keep at least 3 days (72 hours) between two doses, including when you move your injection day.[1]
- Multi-dose vial or KwikPen: throw it away 30 days after first use or after 4 weekly doses, even if medicine is left.[1]
Before a late injection
- The dose record shows this dose was not already given
- Hours counted from the scheduled time
- Eating and drinking normally, with no ongoing vomiting
- No instruction from your clinician to hold Zepbound for now
- Your next regular dose will still be at least 72 hours after this one

Get help before working out any timing
- Swelling of the face, lips, tongue or throat, or trouble breathing or swallowing: emergency care.[1]
- Severe stomach pain that does not go away, sometimes reaching the back, with or without vomiting.[1]
- You may have injected twice or used too much: call the Poison Help Line on 1-800-222-1222.[1]
For the pharmacist
- Is my device single-dose or multi-dose, and does it have a use-by limit once opened?
- If I skip a week, what do I do with a dose still left at day 30?
- Can I move my regular injection day, and which day should I choose?
