Mounjaro and Gallbladder Disease: Diet Protocols That Help
At a glance
- Label rate for acute gallbladder disease / 0.6% on tirzepatide vs 0% on placebo in adult placebo-controlled trials [2]
- If stones are suspected / label states gallbladder diagnostic studies and clinical follow-up are indicated [2]
- Fiber and carbohydrates / more high-fiber foods, fewer refined carbohydrates and less sugar [1]
- Fats / olive oil and fish oil favored; fried foods and dessert fats limited [1]
- Rapid weight loss / a modifiable risk factor for cholesterol gallstones [3]
- Ursodiol evidence / gastric bypass trial: 2% on 600 mg daily vs 32% on placebo at 6 months [4]
How common is gallbladder disease on Mounjaro?
The Mounjaro label lists acute gallbladder disease as a warning. In adult placebo-controlled trials, acute gallbladder disease (cholelithiasis, biliary colic and cholecystectomy) was reported by 0.6% of tirzepatide-treated patients and 0% of placebo-treated patients [2]. A meta-analysis of nine tirzepatide trials with 9,871 participants found a higher composite of gallbladder or biliary disease with tirzepatide than with placebo or basal insulin (RR 1.97, 95% CI 1.14 to 3.42), but not a significant increase in cholelithiasis, cholecystitis or biliary disease considered separately [5].
What diet changes may help?
NIDDK's guidance on preventing gallstones recommends more high-fiber foods such as fruits, vegetables, beans, peas and whole grains including brown rice, oats and whole wheat bread; fewer refined carbohydrates and less sugar; and healthy fats such as fish oil and olive oil, which it says help the gallbladder contract and empty regularly. It advises avoiding unhealthy fats such as those in desserts and fried foods, and says very low-calorie diets can cause rapid weight loss and raise gallstone risk [1].
A cohort of 1,962 adults in southern Italy, followed for about seven years, found that eating fried foods and excessive oil was associated with more new gallstones, while fish and wholemeal bread were associated with fewer [6]. That cohort links foods with stone risk; it did not test a prevention diet.
Does weight-loss speed matter?
Rapid weight loss is one of the modifiable risk factors for cholesterol gallstones, along with obesity and a sedentary lifestyle [3]. Mounjaro is labeled for type 2 diabetes, not weight loss, but weight does fall on it: in the 40-week SURPASS-2 trial, adults on tirzepatide lost 7.6 to 11.2 kg depending on dose, versus 5.7 kg on semaglutide 1 mg [7]. We found no study that tied the speed of weight loss on Mounjaro to gallbladder events.
Ursodiol: what the evidence covers
In a gastric bypass trial, 233 patients had at least one postoperative ultrasound. Gallstones had formed at 6 months in 32% on placebo, 13% on ursodiol 300 mg, 2% on 600 mg and 6% on 1,200 mg daily [4]. The authors concluded that 600 mg daily is effective prophylaxis after gastric-bypass-induced rapid weight loss [4]. That trial was in gastric bypass patients, not people taking Mounjaro.
