Testosterone Cypionate Acne: Alternatives Without This Side Effect
At a glance
- Acne on cypionate / Listed among skin and appendage reactions in the Depo-Testosterone label, with no frequency given [1]
- Hormone curve after 200 mg intramuscular cypionate / Serum testosterone rose about threefold, peaking on days 2 to 5 [2]
- Steady route in a 24-week comparison / A nightly patch kept testosterone and estradiol in the normal range; every-2-week injections ran supraphysiological for days [3]
- Weekly low-dose subcutaneous shot / Peak and trough both in the normal range in 22 men, in a pilot of testosterone enanthate [4]
- Acne treatment reviewed / Topical retinoid, with benzoyl peroxide added [5]
- What no route avoids / FDA says blood pressure rises with all testosterone products, class-wide [6]
Does testosterone cypionate cause acne?
Yes. Acne appears in the Depo-Testosterone label's list of skin and appendage reactions, alongside hirsutism, male pattern baldness and seborrhea, and the label gives no frequency for any of them [1]. Testosterone cypionate is an FDA-approved intramuscular injection for men with hypogonadism, dosed 50 to 400 mg every 2 to 4 weeks [1]. HealthRX.com offers testosterone cypionate by prescription.
Increased sebum secretion is considered the major factor in the pathophysiology of acne [7]. Acne is also one of the most common adverse effects for many transmasculine patients receiving masculinizing testosterone therapy [8], and a review of testosterone therapy for transgender men lists acne among the undesired effects [9].
What the cypionate injection curve looks like
Cypionate is an oil-soluble ester. Injected into muscle, it is absorbed slowly from the oil depot and splits to free testosterone, with a half-life of about 8 days [1]. Slow release still leaves a visible curve in the blood.
In 11 hypogonadal men given 200 mg of intramuscular testosterone cypionate, serum testosterone rose about threefold, peaked on days 2 to 5, and fell back to baseline by days 13 to 14. The authors concluded that 200 mg every 2 weeks produces wide swings, from high to basal [2]. Cypionate produced a marked hormone peak in that small study, but the study did not measure whether the peak affected acne.
What changes in the blood when you switch
Route changes the shape of the hormone curve measurably, even where skin was not studied.
In a 24-week randomized comparison of 66 hypogonadal men, intramuscular testosterone enanthate 200 mg every 2 weeks produced supraphysiological total testosterone, bioavailable testosterone and estradiol (but not dihydrotestosterone) for several days after each injection. A nightly testosterone patch produced circadian variation that stayed within normal physiological ranges, and mean morning hormone levels fell in the normal range in a greater proportion of patch users (77% to 100%) than injection users (19% to 84%) [3].
A smaller option is weekly low-dose subcutaneous injection. In a 2006 pilot of 22 hypogonadal men self-injecting testosterone enanthate weekly with an insulin syringe, peak and trough levels were both within the normal range in all 22, and no local reactions were reported [4]. That was a single small pilot of enanthate, not cypionate.
One thing a switch does not avoid: FDA says ambulatory blood pressure studies confirmed that blood pressure increases with all testosterone products, class-wide [6].
What the studies do not show
The studies above compare hormone levels, not acne rates. The cypionate pharmacokinetic study used 200 mg every two weeks, not split doses [2]. None of the formulation changes above can be ranked against cypionate for acne.
Treating the acne while staying on cypionate
Topical retinoids are first-line treatment for acne vulgaris and performed better than vehicle in the trials reviewed [5]. In a systematic review of 54 clinical trials, a topical retinoid combined with benzoyl peroxide performed better than vehicle at week 12, with Investigator Global Assessment improvement in 26.1% to 34.9% versus 7% to 11.8% [5].
Tolerability differs by agent and strength. In the same review, adverse events were reported by 62% of patients on tretinoin 0.05%, 40% on adapalene 0.3% and 19% on adapalene 0.1% [5]. The American Academy of Dermatology guideline covers acne grading and both topical and systemic management [10].
For severe or refractory acne, a practical guide for transmasculine patients on testosterone reviews isotretinoin indications, contraindications and iPLEDGE barriers [8]. That guide is written for masculinizing hormone therapy in transmasculine patients, not for men treated for hypogonadism.
Non-testosterone options: clomiphene
Clomiphene citrate has been studied as a way to raise testosterone in men with hypogonadism. In a series of 46 men with hypogonadism (mean age 44) treated for over 12 months, mean serum testosterone rose from 228 ng/dL at baseline to 612 ng/dL at 1 year, 562 ng/dL at 2 years and 582 ng/dL at 3 years (P<0.001), and the authors reported no adverse events from any patient [12]. The series tracked hormone levels, bone density and symptom scores.
