Testosterone Cypionate Fertility Suppression That Doesn't Resolve
At a glance
- What suppresses sperm / Testosterone feedback-inhibits pituitary LH and, at large doses, FSH [3]
- Typical recovery / 67% by 6 months and 90% by 12 months to 20 million/mL in 1,549 men on hormonal contraception [1]
- Depo-Testosterone label / Lists oligospermia at high dosages; subfertility and infertility reported with anabolic steroid abuse [3]
- hCG-based therapy / Spermatogenesis returned or improved in 47 of 49 men (95.9%), mean 4.6 months [4]
- hCG plus a SERM / 70% reached a total motile count above 5 million within 12 months [2]
- Slower recovery / Older age and longer testosterone use in the 66-man series [2]
- Monitoring sperm production / Semen analysis; a serum testosterone level measures something different [5]
How testosterone cypionate shuts down sperm production
The Depo-Testosterone label describes the mechanism: exogenous androgens inhibit pituitary luteinizing hormone (LH) release through feedback, and at large doses of exogenous androgens spermatogenesis may also be suppressed through feedback inhibition of pituitary follicle stimulating hormone (FSH), possibly lowering sperm count [3]. The label gives a half-life of about 8 days after an intramuscular injection [3].
A serum testosterone level in the normal range does not show whether sperm production has returned. Semen analysis is the test used to monitor spermatogenesis during and after suppression of fertility [5].
What recovery usually looks like after stopping
The largest timed dataset is an integrated analysis of 30 hormonal male contraception studies, published in the Lancet in 2006, covering 1,549 healthy men aged 18 to 51 [1]. Median time to recover a sperm concentration of 20 million/mL was 3.4 months (95% CI 3.2 to 3.5) [1]. Recovery to that threshold was 67% by 6 months, 90% by 12 months, 96% by 16 months and 100% by 24 months [1].
Faster recovery was associated with shorter-acting testosterone preparations, shorter treatment duration, higher baseline sperm concentration and faster initial suppression [1]. Older age was also associated with faster recovery in that analysis [1].
One limit applies. The participants were healthy men whose sperm production was suppressed on purpose for contraception, not men treated for hypogonadism with cypionate injections, so the percentages are a guide rather than a prediction for one person [1].
What hCG-based treatment studies found
A 2015 retrospective series reviewed 49 men who had azoospermia or severe oligospermia (under 1 million sperm/mL) while taking exogenous testosterone [4]. They received hCG 3,000 units subcutaneously every other day, supplemented with clomiphene, tamoxifen, anastrozole or recombinant FSH according to physician preference [4]. In this uncontrolled case series, spermatogenesis returned or improved in 47 men (95.9%), at an average of 4.6 months, with a mean first sperm density of 22.6 million/mL, and there was no significant difference in recovery by type of testosterone used [4].
A 2017 retrospective study followed 66 men who presented with infertility after testosterone use; testosterone was stopped and they received high-dose hCG plus a selective estrogen receptor modulator [2]. A total motile count above 5 million within 12 months was reached by 46 men (70%) [2]. Among azoospermic men, 64.8% reached it, compared with 91.7% of cryptozoospermic men [2]. Older age and longer testosterone use each correlated with longer time to recovery [2].
The 49-man series does not establish whether testosterone continued during hCG-based treatment [4]. Neither series had a control group, so neither shows how many of these men would have recovered without treatment [4][2].
Does hCG taken during testosterone therapy prevent the problem?
In a retrospective review of 26 hypogonadal men who stayed on testosterone (19 on injections, 7 on gel) and added hCG 500 IU every other day, no patient became azoospermic [9]. No differences in semen parameters were observed during follow-up of more than a year, and 9 of the 26 men contributed to a pregnancy with their partner [9]. That was a small review without a comparison group, so it cannot show how much of the result is due to hCG.
A 2014 review of anabolic steroid-induced hypogonadism notes that symptomatic hypogonadism can follow anabolic steroid use and depends on dose, duration and type; it lists management options as judicious testosterone replacement, hCG and selective estrogen receptor modulators [7].
What the label says about fertility
The Depo-Testosterone label lists oligospermia at high dosages among its adverse reactions [3]. Its drug abuse section notes that testicular atrophy, subfertility and infertility have been reported in men who abuse anabolic steroids, typically at doses above those recommended and often with other anabolic androgenic steroids [3].
If you have not started testosterone cypionate yet
The most direct statement is the Endocrine Society's 2018 guideline, which recommends against starting testosterone therapy in men planning fertility in the near term [8].
