Why Testosterone Cypionate Causes Accelerated Male-Pattern Hair Loss: The Mechanism Explained

At a glance

  • Label status / Male pattern baldness is listed as an adverse reaction [1]
  • Hormone pathway / Testosterone activity in many tissues depends on conversion to DHT [1]
  • AGA biology / DHT drives miniaturization of androgen-sensitive follicles [2][3]
  • Genetic gate / Androgen receptor gene variants are the major determinant of early-onset AGA [4]
  • Timing / A review of testosterone therapy in transgender men reports that alopecia continues to develop after 1 year [9]
  • Approved AGA drugs / Minoxidil and finasteride are FDA-approved for androgenetic alopecia [2]

Does the label list hair loss?

Yes. The Depo-Testosterone label lists male pattern of baldness under skin and appendages adverse reactions [1]. It gives no percentage, time to onset, dose threshold or reversal statement [1]. A 2017 review of testosterone therapy in transgender men lists alopecia among the undesired effects and notes that most testosterone effects start to develop within several months, although facial hair and alopecia continue to develop after 1 year [9].

Where does DHT fit?

The label says testosterone activity in many tissues appears to depend on reduction to DHT [1]. DHT is made from testosterone by 5-alpha reductase [5]. In androgenetic alopecia, DHT acts on androgen-sensitive scalp follicles; reviews describe progressive reduction in hair diameter, length and pigmentation, and miniaturization with a shorter anagen phase and longer telogen phase [6][3].

Why do only some men lose hair?

Genetic susceptibility is the main requirement. A 2005 study found androgen receptor gene variability was the major determinant of common early-onset AGA, with an etiological fraction of 0.46 [4]. The AR gene is X-linked, which the authors say stresses the importance of the maternal line in inheritance [4]. An earlier study found the AR StuI restriction site in 98.1% of 54 young bald men versus 76.6% of nonbald men [7].

What about finasteride and minoxidil?

Minoxidil and finasteride are FDA-approved for androgenetic alopecia, and a meta-analysis found both superior to placebo for hair outcomes in men [2]. Dutasteride inhibits both type 1 and type 2 5-alpha reductase and is not FDA-approved for AGA [5]. A 24-week trial of topical finasteride in 458 men with AGA found a target area hair count change of 20.2 hairs versus 6.7 with placebo, and serum DHT fell 34.5% with topical versus 55.6% with oral finasteride [8]. These trials studied men with androgenetic alopecia; they do not report whether finasteride changes hair loss specifically in men taking testosterone cypionate [8][2].

Does hair loss reverse after stopping cypionate?

The Depo-Testosterone label does not address whether hair grows back after stopping [1]. Androgenetic alopecia is a progressive condition that tends to worsen with time [3].

Frequently asked questions

Does testosterone cypionate cause male-pattern hair loss?
The Depo-Testosterone label lists male pattern baldness as an adverse reaction [1]. It gives no incidence rate or typical time to onset.
How does testosterone lead to DHT?
The label says testosterone activity in many tissues depends on reduction to dihydrotestosterone [1]. DHT is made from testosterone by 5-alpha reductase [5].
Can finasteride prevent hair loss on testosterone cypionate?
Finasteride is FDA-approved for androgenetic alopecia and was superior to placebo in a meta-analysis of men with the condition [2]. Those trials were not designed around men taking testosterone cypionate [8][2].
Does hair grow back after stopping testosterone cypionate?
The label does not address hair recovery after stopping [1]. Androgenetic alopecia tends to worsen with time [3].
