Wegovy Delayed-Onset Side Effects: What Happens Weeks or Months After Starting
At a glance
- Drug / semaglutide 2.4 mg (Wegovy), once-weekly injection
- Maintenance dosing / begins at week 17 after four 4-week steps; the maintenance dose depends on the indication [1]
- Gallstones / 1.6% vs 0.7% placebo in adult weight trials [1]
- Cholecystitis / 0.6% vs 0.2% placebo [1]
- Pancreatitis / 4 Wegovy-treated adults (0.2 per 100 patient-years) vs 1 on placebo [1]
- Hair loss / 3% vs 1%; 6% at the 7.2 mg dose [1]
- Altered skin sensation / 2% vs 1% at 2.4 mg; 22% vs 0% at 7.2 mg [1]
- Resting heart rate / mean rise of 1 to 4 beats per minute [1]
What does 'delayed' mean for Wegovy?
Wegovy is built up slowly: 0.25 mg weekly for weeks 1 to 4, 0.5 mg for weeks 5 to 8, 1 mg for weeks 9 to 12, 1.7 mg for weeks 13 to 16, then maintenance dosing from week 17 [1]. The trial figures report how often each reaction occurred during treatment, not when it first appeared [1].
In the pooled adult weight-management trials, gastrointestinal reactions were reported by 73% of patients on Wegovy 2.4 mg versus 47% on placebo, and 6.8% versus 3.2% stopped treatment because of an adverse reaction [1]. The adult 2.4 mg injection adverse-reaction table covers trials lasting up to 68 weeks; other findings below use different measures or trial durations [1].
Gallbladder disease
In the adult weight-management trials, cholelithiasis was reported by 1.6% of patients on Wegovy injection versus 0.7% on placebo, and cholecystitis by 0.6% versus 0.2% [1]. The Wegovy tablets had cholelithiasis in 2.5% versus 1% on placebo, and in the adolescent injection trial 3.8% of Wegovy-treated patients reported it versus 0% on placebo [1].
The label says substantial or rapid weight loss can increase the risk of cholelithiasis, but the incidence of acute gallbladder disease was greater in Wegovy-treated patients than in placebo-treated patients even after accounting for the degree of weight loss. If cholelithiasis is suspected, it says gallbladder studies and appropriate clinical follow-up are indicated [1].
Pancreatitis
Acute pancreatitis was confirmed in four Wegovy-treated adults, or 0.2 per 100 patient-years, versus one on placebo [1]. The label describes what to watch for: persistent or severe abdominal pain, sometimes radiating to the back, which may or may not come with nausea or vomiting. It says to discontinue Wegovy if pancreatitis is suspected and to begin appropriate management [1].
Average lipase rose 39% and amylase 15% to 16% in Wegovy trials. The label says the significance of these increases without pancreatitis symptoms is unknown [1].
Kidney injury from dehydration
Wegovy's label warns about acute kidney injury associated with dehydration, often following nausea, vomiting or diarrhea [1]. Acute kidney injury occurred at 0.4 versus 0.2 cases per 100 patient-years (7 versus 4 adults), more often in patients with existing kidney impairment, during dose titration, and after gastrointestinal fluid loss [1]. The label says to monitor kidney function in patients reporting reactions that could lead to volume depletion [1].
Hair loss
Hair loss is in the label's adverse reaction table: 3% on Wegovy 2.4 mg versus 1% on placebo, and it is on the label's list of most common adverse reactions (incidence of 5% or more) in adults or pediatric patients aged 12 and older [1]. In the higher-dose program, hair loss was reported by 6% on 7.2 mg versus 3% on 2.4 mg and 1% on placebo, and affected 8.4% of women versus 0.2% of men on 7.2 mg [1].
Altered skin sensation (dysesthesia)
Dysesthesia covers altered skin sensation such as burning or pain from touch, and it was reported by 2% on Wegovy 2.4 mg versus 1% on placebo [1]. At 7.2 mg weekly the gap widens sharply: 22% versus 6% on 2.4 mg and 0% on placebo. The label says dysesthesia rises with dose, and 18% of those affected on 7.2 mg had not recovered by the end of the trial [1].
Heart rate
Mean resting heart rate rose 1 to 4 beats per minute on Wegovy versus placebo, and a rise of 20 beats per minute or more occurred in 26% versus 16% [1]. The label lists heart rate increase as a warning and says to monitor heart rate at regular intervals [1].
Eyes: retinopathy and a rare optic nerve condition
In a two-year trial in people with type 2 diabetes and high cardiovascular risk, retinopathy complications occurred in 3% on semaglutide 0.5 or 1 mg versus 1.8% on placebo, and in 8.2% versus 5.2% of those who already had retinopathy at baseline [1]. Rapid improvement in glucose control has been associated with temporary worsening of retinopathy, and the Wegovy label says patients with a history of diabetic retinopathy should be monitored [1].
Europe's medicines safety committee concluded in June 2025 that NAION, an optic nerve condition that can cause vision loss, is a very rare side effect of semaglutide, meaning up to 1 in 10,000 people. Its estimate was about one extra case per 10,000 person-years, roughly a two-fold relative risk in large studies of adults with type 2 diabetes, and it recommended stopping semaglutide if NAION is confirmed [2]. The current US Wegovy label does not mention NAION [1].
Bowel effects and anesthesia
The Wegovy label says pulmonary aspiration has been reported in patients receiving GLP-1 receptor agonists who underwent elective surgeries or procedures with general anesthesia or deep sedation, and tells prescribers to instruct patients to inform healthcare providers of any planned surgeries or procedures [1]. The postmarketing list includes ileus, intestinal obstruction and severe constipation with fecal impaction, for which a frequency cannot be estimated from voluntary reports [1]. The label also says Wegovy is not recommended in people with severe gastroparesis, while not stating that the drug causes gastroparesis [1].
How long Wegovy stays in the body
Semaglutide's elimination half-life is about one week, and the drug stays in the body for roughly 5 to 7 weeks after the last 2.4 mg or 7.2 mg injection or 25 mg tablet [1].
