At a glance

  • Drug / semaglutide 2.4 mg (Wegovy), once-weekly subcutaneous injection
  • Weight change after switching to placebo / +6.9% vs -7.9% for continuing, weeks 20 to 68 (STEP 4, n=803) [2]
  • Weight regained one year after stopping / 11.6 percentage points, about two-thirds of the loss (STEP 1 extension, n=327) [3]
  • How long the drug lasts / about 5 to 7 weeks after the last 2.4 mg injection [1]
  • Most common reactions while taking it / nausea 44% vs 16%, diarrhea 30% vs 16%, vomiting 24% vs 6%, constipation 24% vs 11% [1]
  • Serious GI reactions / 4.1% vs 0.9% on placebo [1]

Does stopping Wegovy cause withdrawal?

Semaglutide is a GLP-1 receptor agonist. It selectively binds and activates the GLP-1 receptor, including receptors in brain areas that regulate appetite and calorie intake, and it delays gastric emptying [1].

The label's warnings section covers acute pancreatitis, acute gallbladder disease, hypoglycemia when Wegovy is used with insulin or an insulin secretagogue, acute kidney injury from volume depletion, severe gastrointestinal reactions, hypersensitivity reactions, diabetic retinopathy complications, heart rate increase, and pulmonary aspiration during general anesthesia or deep sedation. A withdrawal or dependence reaction is not among them [1].

After the last injection, the change that the trials documented is weight regain: in STEP 4 and in the STEP 1 extension, weight came back once treatment stopped [3][2].

What the label says about stopping and about missed doses

The label gives no tapering schedule. What it does address is interrupted dosing: the Wegovy injection label says a missed weekly dose is given if the next scheduled dose is more than 2 days away, and after 2 or more consecutive missed doses it describes restarting at a lower dose and increasing again [1].

The label also covers planned stopping around pregnancy. For weight reduction or cardiovascular risk reduction, it says to discontinue Wegovy when pregnancy is recognized, and to stop at least 2 months before a planned pregnancy because of the long half-life of semaglutide [1].

Situations where the label or a regulator says to stop

  • If pancreatitis is suspected, the label says to discontinue Wegovy and start appropriate management [1].
  • If a serious hypersensitivity reaction such as anaphylaxis or angioedema occurs, the label says to discontinue Wegovy [1].
  • In Europe, EMA's safety committee concluded in June 2025 that NAION, a sudden loss of vision from optic nerve damage, is a very rare side effect of semaglutide, may affect up to 1 in 10,000 people, and advised stopping semaglutide if NAION is confirmed [4]. NAION does not appear among the Wegovy label's warnings [1].

How long semaglutide stays in the body

The elimination half-life is about 1 week, and the label says semaglutide stays in the body for about 5 to 7 weeks after the last 2.4 mg or 7.2 mg injection or 25 mg tablet [1]. The label gives this long half-life as the reason for the 2-month gap before a planned pregnancy [1].

Weight regain after stopping: STEP 4 and the STEP 1 extension

STEP 4 was a randomized withdrawal study. After a 20-week run-in during which participants reached the 2.4 mg dose and lost a mean 10.6% of body weight, 803 adults were randomized to keep taking semaglutide 2.4 mg (n=535) or switch to placebo (n=268) for 48 more weeks. From week 20 to week 68, the continuing group lost a further 7.9% of body weight while the placebo group gained 6.9%, a difference of 14.8 percentage points [2].

The longer view comes from an off-treatment extension of STEP 1, which followed 327 participants for a year after treatment ended. Mean weight loss at week 68 had been 17.3% on semaglutide versus 2.0% on placebo. One year after stopping, the semaglutide participants had regained 11.6 percentage points of body weight, about two-thirds of what they had lost, leaving a net loss of 5.6% from baseline versus 0.1% for placebo [3].

Most of the cardiometabolic improvements seen on treatment moved back toward baseline during that year [3]. STEP 4 also tracked waist circumference, systolic blood pressure and physical functioning after participants switched to placebo [2].

Frequently asked questions

Does stopping Wegovy cause withdrawal symptoms?
The label's warnings do not list a withdrawal or dependence reaction [1]. The trial evidence on stopping is about weight: in STEP 4, switching to placebo after 20 weeks on semaglutide produced a 6.9% weight gain over 48 weeks, against a further 7.9% loss in people who continued on 2.4 mg [2].
How much weight comes back after stopping Wegovy?
In STEP 4, adults who switched to placebo gained 6.9% of body weight between weeks 20 and 68, while those who continued on 2.4 mg lost another 7.9% [2]. In a 327-person extension of STEP 1, people who stopped regained 11.6 percentage points in the year after treatment ended, about two-thirds of what they had lost, for a net loss of 5.6% from baseline [3].
How long does semaglutide stay in the body after the last dose?
The elimination half-life is about 1 week, and the label says semaglutide stays in the body for about 5 to 7 weeks after the last 2.4 mg or 7.2 mg injection or 25 mg tablet [1].
Do you need to taper off Wegovy?
The Wegovy label gives no taper schedule [1]. For interrupted dosing, it describes giving the missed weekly dose if the next scheduled dose is more than 2 days away, and restarting at a lower dose after 2 or more consecutive missed doses [1].
What happens to cardiometabolic measures after stopping?
In the STEP 1 extension, cardiometabolic measures that improved on semaglutide mostly moved back toward baseline in the year after stopping [3]. STEP 4 also measured waist circumference and systolic blood pressure after participants switched to placebo [2].
When does the label say to stop Wegovy?
The label says to discontinue if pancreatitis is suspected, and to discontinue for a serious hypersensitivity reaction such as anaphylaxis or angioedema [1]. For weight reduction or cardiovascular risk reduction it also says to discontinue when pregnancy is recognized and to stop at least 2 months before a planned pregnancy [1].
