Wegovy injection-site reactions: comfort measures, medicine limits and warning signs

At a glance

  • Reported rate / 1.4% with Wegovy 2.4 mg vs 1% with placebo in pooled adult trials [1]
  • Where to inject / Abdomen, thigh or upper arm; the site can change without a dose change [1]
  • Itch cream / MedlinePlus: hydrocortisone topical treats redness, swelling, itching and discomfort of various skin conditions [2]
  • Antihistamine / MedlinePlus: cetirizine treats hives and itching, can cause drowsiness, and is not a substitute for an epinephrine injector [3]
  • Label action / Suspected hypersensitivity: discontinue Wegovy [1]
  • Emergency signs listed for hives / Trouble breathing, speaking or swallowing, mouth or tongue swelling, wheezing, dizziness, loss of consciousness [3]

How common is an injection-site reaction with Wegovy?

In the label's pooled weight-management trials, 2,116 adults received Wegovy 2.4 mg weekly and 1,261 received placebo for up to 68 weeks. Injection-site reactions were reported by 1.4% of the Wegovy group and 1% of the placebo group [1]. That is a whole-trial figure; it says nothing about how severe any one person's next reaction will be.

Wegovy is injected under the skin of the abdomen, thigh or upper arm, once weekly on the same day each week, at any time of day, with or without meals. The label says the time of day and the injection site can be changed without a dosage change [1].

What does the label say about technique?

Most of the label's practical content concerns technique rather than aftercare. It says patients should be trained on the exact Wegovy presentation they receive, single-dose pen or syringe or the FlexTouch pen, and should follow the Instructions for Use for that presentation [1]. Wegovy's injection instructions say the solution should be inspected before each dose and used only if it is clear, colorless and free of particles [1]. We found no study or label statement on cooling, moisturizers or clothing for injection-site reactions.

Can hydrocortisone cream or an antihistamine help?

MedlinePlus describes topical hydrocortisone as used to treat redness, swelling, itching and discomfort of various skin conditions, and says to stop using an over-the-counter product if the condition has not improved within 7 days [2]. It lists severe rash, and redness, swelling or other signs of skin infection where hydrocortisone was applied, among its serious side effects [2].

MedlinePlus describes cetirizine as an antihistamine used for allergy symptoms and for urticaria, meaning hives or itchy raised red bumps [3]. It says cetirizine may cause drowsiness, that alcohol can add to that drowsiness, and that hives which do not improve in the first 3 days, or last longer than 6 weeks, are a reason to stop it [3].

Neither product has been studied as a treatment for semaglutide injection-site reactions, so what each page covers is general drug information.

Which symptoms point to a serious allergic reaction?

Wegovy's warnings section lists hypersensitivity: anaphylactic reactions and angioedema have been reported after marketing, and the label says to discontinue Wegovy if a hypersensitivity reaction is suspected [1]. A prior serious hypersensitivity reaction to semaglutide or to any excipient in Wegovy is a contraindication to using it at all [1].

The MedlinePlus cetirizine page names the symptoms of a life-threatening allergic reaction in people treating hives and says to get emergency medical help right away for any of them: difficulty swallowing, speaking or breathing; swelling in and around the mouth or swelling of the tongue; wheezing; drooling; dizziness; or loss of consciousness [3]. It also says cetirizine must not be used in place of an epinephrine injector [3].

Allergic reactions also appeared in the Wegovy trials themselves: 16% of patients who developed anti-semaglutide antibodies reported an allergic reaction, versus 7% of those who did not [1].

Should an antihistamine be taken before the next injection?

We found no study or label statement that recommends an antihistamine before a Wegovy injection. The label's hypersensitivity guidance is to discontinue Wegovy if a reaction is suspected [1].

What about compounded semaglutide?

Compounded semaglutide is not FDA approved and is not reviewed for safety, effectiveness or quality before it is marketed, and FDA has reported dosing errors with compounded injectable semaglutide, including patients measuring the wrong dose [4]. It is a different product from Wegovy, so the trial figure above does not describe it. HealthRX.com offers compounded semaglutide by prescription.

Frequently asked questions

Can I take an antihistamine before my next Wegovy dose?
We found no study or label statement recommending it. The label's hypersensitivity guidance is to discontinue Wegovy if a hypersensitivity reaction is suspected [1]. MedlinePlus says cetirizine relieves hives and itch and must not be used in place of an epinephrine injector [3].
How long can I use hydrocortisone cream on a reaction?
MedlinePlus says that if an over-the-counter hydrocortisone product has not improved the condition within 7 days, use should stop, and it lists severe rash, or redness, swelling or other signs of skin infection where it was applied, among serious side effects [2].
Can I put ice on the injection site?
We found no study or label statement on ice or cooling for Wegovy injection-site reactions. For a suspected hypersensitivity reaction, the label says to discontinue Wegovy [1].
Should I stop Wegovy because of a red spot at the injection site?
The label says to discontinue Wegovy if a hypersensitivity reaction is suspected [1]. It also lists a prior serious hypersensitivity reaction to semaglutide or an excipient as a contraindication [1]. Redness alone does not say which kind of reaction it is; injection-site reactions in the trials were reported by 1.4% on Wegovy versus 1% on placebo [1].
