Using Dose Titration to Resolve Sulfur Burps on Wegovy (Semaglutide 2.4 mg)

At a glance

  • Labeled belching rate / 7% on Wegovy 2.4 mg versus under 1% on placebo [1]
  • Labeled titration / 0.25, 0.5, 1, 1.7 mg every 4 weeks, then 2.4 mg from week 17 [1]
  • Maintenance / 2.4 mg recommended or 1.7 mg for weight reduction in adults [1]
  • Missed doses / Label says to give the dose as soon as possible if the next dose is more than 2 days away; after 2 or more consecutive missed doses, reinitiate escalation at a lower dosage [1]
  • Half-life / About 1 week; drug remains 5 to 7 weeks after the last 2.4 mg injection [1]

What the Wegovy label reports about belching

The label's pooled safety data cover 2,116 adults on Wegovy 2.4 mg weekly and 1,261 on placebo for up to 68 weeks. Eructation was reported by 7% on Wegovy and under 1% on placebo. Any gastrointestinal reaction occurred in 73% versus 47% [1]. Eructation is the label's term for belching; the label notes separately that Wegovy delays gastric emptying [1].

The labeled titration, step by step

Wegovy injection is given once weekly, on the same day each week, at any time of day, with or without meals [1]. The labeled climb for adults is 0.25 mg weeks 1 to 4, 0.5 mg weeks 5 to 8, 1 mg weeks 9 to 12, 1.7 mg weeks 13 to 16, then maintenance from week 17 at 2.4 mg (recommended) or 1.7 mg [1]. Adults who tolerate 2.4 mg for at least 4 weeks and need more weight loss may increase to a maximum of 7.2 mg weekly [1].

Can you stretch a step or pause?

The labeled injection schedule increases the dose every 4 weeks. If a dose is not tolerated during escalation, the label says to consider delaying escalation for 4 weeks [1]. We found no study testing whether a delay or a longer stay at one step reduces sulfur burps.

For a missed weekly dose, the Wegovy label says to administer it as soon as possible if the next scheduled dose is more than 2 days away, and not to administer it if the next dose is less than 2 days away. After 2 or more consecutive missed doses, it says to reinitiate dosage escalation at a lower dosage [1]. The half-life is about 1 week, and the drug remains in the body roughly 5 to 7 weeks after the last 2.4 mg injection [1].

Is 1.7 mg a real maintenance dose?

Yes. The label lists 2.4 mg (recommended) or 1.7 mg for weight reduction in adults [1].

What the label says to watch for

  • Severe gastrointestinal reactions were reported in 4.1% on Wegovy versus 0.9% on placebo [1].
  • The label says Wegovy is not recommended in patients with severe gastroparesis, and postmarketing reports include ileus and intestinal obstruction [1].
  • Acute kidney injury from volume depletion occurred in 0.4 versus 0.2 cases per 100 patient-years, more often during dose titration [1].

Frequently asked questions

Does the label describe a way to slow the titration?
The labeled schedule advances every 4 weeks from 0.25 mg to 2.4 mg, and the label says to consider delaying escalation for 4 weeks if a dose is not tolerated [1].
How long does Wegovy stay in the body if I pause?
The half-life is about 1 week, and semaglutide remains in the body roughly 5 to 7 weeks after the last 2.4 mg injection [1].
Do the tablets follow the same schedule?
No. Wegovy tablets are taken once daily on an empty stomach with up to 4 ounces of plain water, swallowed whole, with at least 30 minutes before food or drink. The steps are 1.5 mg for 30 days, 4 mg for days 31 to 60, 9 mg for days 61 to 90, then 25 mg [1].
