Cialis (Tadalafil) Food and Supplement Interactions: What to Avoid and Why

At a glance

  • Food effect / Cialis may be taken with or without food [1]
  • Half-life / 17.5 hours; erectile function improved versus placebo for up to 36 hours after a dose [1]
  • Main metabolic route / CYP3A4 [1]
  • Absolute contraindication / any organic nitrate, used regularly or intermittently [1]
  • Grapefruit juice / would likely increase tadalafil exposure [1]
  • Alcohol / 5 units or more can add orthostatic symptoms such as dizziness and headache [1]
  • Once-daily dosing / continuous plasma levels, which the label says to weigh when assessing interactions [1]
  • Nitrate emergency rule / at least 48 hours after the last Cialis dose, with hemodynamic monitoring [1]

Does food change tadalafil absorption?

No. The Cialis label states that Cialis may be taken without regard to food, and that the rate and extent of absorption are not influenced by food [1]. Peak plasma concentration arrives between 30 minutes and 6 hours after a dose, with a median of 2 hours [1].

What you eat with the tablet is a separate question from what else is in your body. The interactions below come from enzyme inhibition, added vasodilation, or an overlapping drug class.

Grapefruit juice: a likely interaction that was never measured directly

Tadalafil is metabolized mainly by CYP3A4 [1]. The label names grapefruit juice among the CYP3A4 inhibitors that, although not studied with tadalafil, would likely increase exposure [1]. Where inhibitors of this enzyme have been tested, the effect was large: ketoconazole 400 mg daily raised tadalafil 20 mg exposure (AUC) by 312% and peak concentration by 22%, ketoconazole 200 mg daily raised tadalafil 10 mg AUC by 107%, and ritonavir 200 mg twice daily raised tadalafil 20 mg AUC by 124% [1].

The Cialis label sets dose ceilings for people taking CYP3A4 inhibitors: as-needed use no more than 10 mg every 72 hours, and once-daily use no more than 2.5 mg [1]. The label does not quantify the grapefruit effect or give a timing window.

Alcohol: the label's 5-unit threshold

Both alcohol and tadalafil are mild vasodilators, and when mild vasodilators are combined the blood-pressure-lowering effect of each can increase [1]. The label ties a specific threshold to symptoms: substantial alcohol, meaning 5 units or more, with Cialis can increase orthostatic signs and symptoms, including higher heart rate, lower standing blood pressure, dizziness and headache [1]. Plasma levels are not the issue; tadalafil did not change alcohol concentrations and alcohol did not change tadalafil concentrations [1].

For scale, tadalafil 20 mg alone lowered supine blood pressure by a mean maximal 1.6/0.8 mm Hg compared with placebo in healthy volunteers [1]. Once-daily Cialis keeps tadalafil in the blood continuously, and the label lists substantial alcohol among the interactions to consider on that basis [1].

Organic nitrates: the one absolute no

Cialis is contraindicated in anyone using any form of organic nitrate, regularly or intermittently, because tadalafil potentiates the hypotensive effect of nitrates [1]. Guanylate cyclase stimulators such as riociguat are contraindicated as well [1].

The label's 48-hour rule is narrow. In a life-threatening situation where a nitrate is medically necessary, at least 48 hours should elapse after the last Cialis dose, and the nitrate should still be given only under close medical supervision with appropriate hemodynamic monitoring [1]. A 150-man crossover study looked at the time course: men took tadalafil 20 mg or placebo daily for 7 days, then sublingual nitroglycerin 0.4 mg [2]. Standing systolic blood pressure fell below 85 mm Hg in more men on tadalafil at 4, 8 and 24 hours (P<0.05), with no significant difference at 48, 72 and 96 hours [2]. At 48 hours the interaction was not seen by most hemodynamic measures, although a few more men on tadalafil had larger blood pressure falls [1]. The 48-hour figure is an emergency provision, not a routine spacing plan.

Supplements

The closest evidence is for L-arginine, though the trials did not test arginine with tadalafil [3]. A 2019 meta-analysis pooled 10 randomized controlled trials of arginine supplements in 540 men with mild to moderate erectile dysfunction, with doses ranging from 1,500 to 5,000 mg [3]. Arginine improved erectile function compared with placebo or no treatment (odds ratio 3.37, 95% CI 1.29 to 8.77, P=0.01), and adverse events were reported in 8.3% of the arginine group versus 2.3% of the placebo group, none severe [3].

A separate hazard is the product itself. Tadalafil has been identified as an undeclared ingredient in dietary supplements sold for men, in unknown composition and dose [4], and at least 46 analogues of sildenafil, tadalafil and vardenafil have been reported in food supplements, most with no known efficacy or safety profile [5]. A product like that could add an undeclared PDE5 inhibitor to Cialis, and the Cialis label says combinations with other PDE5 inhibitors or other ED treatments were not studied [1].

Enzyme inducers: when exposure falls

Rifampin 600 mg daily reduced tadalafil 10 mg exposure (AUC) by 88% and peak concentration by 46% [1]. Carbamazepine, phenytoin and phenobarbital were not studied but would likely decrease tadalafil exposure [1]. The Cialis label calls for no dose adjustment, but notes that reduced exposure can be anticipated to decrease the efficacy of once-daily Cialis, by an unknown amount [1].

Once daily versus as needed

Once-daily Cialis provides continuous plasma tadalafil levels; steady state is reached within 5 days of daily dosing, with exposure about 1.6-fold that of a single dose [1]. The label's interactions warning says this continuous exposure should be considered when evaluating interactions with nitrates, alpha-blockers, antihypertensives, potent CYP3A4 inhibitors and substantial alcohol [1]. With as-needed dosing, concentrations rise and fall after each tablet.

Cialis is listed as a prescription medicine in the FDA product record [6]. HealthRX.com offers tadalafil by prescription.

Frequently asked questions

Can I drink grapefruit juice with Cialis?
The Cialis label says grapefruit juice would likely increase tadalafil exposure, but the combination was not studied [1]. It gives dose limits for use with CYP3A4 inhibitors such as ketoconazole or ritonavir [1].
Does food affect how well Cialis works?
No. The label says Cialis may be taken without regard to food and that the rate and extent of absorption are not influenced by food [1].
How much alcohol matters with tadalafil?
The label sets the concern at 5 units or more, which can increase orthostatic signs and symptoms such as higher heart rate, lower standing blood pressure, dizziness and headache [1]. Tadalafil did not change alcohol levels and alcohol did not change tadalafil levels [1].
What happens if I take L-arginine with Cialis?
The arginine trials compared arginine with placebo or no treatment, not with tadalafil [3]. In those trials arginine improved erectile function in men with erectile dysfunction [3].
Are beetroot supplements a nitrate risk with Cialis?
The label's contraindication covers any form of organic nitrate used as a medicine [1]. The label does not mention beetroot products, so how they interact with tadalafil is not established.
How long after Cialis can nitroglycerin be given?
In a life-threatening situation the label says at least 48 hours should elapse after the last Cialis dose, and the nitrate should be given only under close medical supervision with hemodynamic monitoring [1]. In a 150-man crossover study, standing systolic pressure fell below 85 mm Hg in more men on tadalafil up to 24 hours after nitroglycerin, with no significant difference at 48 hours [2].
