What does tadalafil actually inhibit?
Tadalafil is a selective PDE5 inhibitor. PDE5 is the enzyme that hydrolyzes cGMP, ending the cGMP signal [2]. In the penis, PDE5 is the predominant phosphodiesterase in the corpus cavernosum [2]. Tadalafil occupies the PDE5 catalytic site and competitively inhibits cGMP hydrolysis, so cGMP rises when nitric oxide is releasing it [2][3].
The label makes the practical point: tadalafil has no effect without sexual stimulation [1].
The pathway: nitric oxide to cGMP to smooth muscle relaxation
During sexual stimulation, nitric oxide is released in the penis. A 1997 review described nitric oxide as the principal mediator of erectile function, with sources including neurons, sinusoidal endothelium, and corporeal smooth muscle cells [4]. Nitric oxide diffuses into smooth muscle cells and binds the heme group of soluble guanylate cyclase, activating the enzyme [5]. Activated guanylate cyclase converts GTP to cGMP [5].
cGMP then acts through several effectors. It activates cGMP-dependent protein kinase (PKG), and it also modulates ion channels and phosphodiesterases [5]. PKG phosphorylates proteins that regulate free intracellular calcium, the cytoskeleton, and the regulatory light chain of smooth muscle myosin [6]. The net effect is lower smooth muscle tone. In the corpus cavernosum, relaxation leads to arterial dilation and venous constriction, producing erection [2].
Tadalafil does not create this signal. It slows the breakdown of cGMP that nitric oxide has already produced. That is why sexual stimulation remains necessary [1].
Why the 36-hour window is not a 36-hour erection
The Cialis label says as-needed tadalafil improved erectile function compared with placebo for up to 36 hours after a dose [1]. That is a window of responsiveness, not a continuous erection. An erection lasting over 4 hours, painful or not, needs emergency care [1].
The half-life helps explain the window. Tadalafil has a mean terminal half-life of 17.5 hours in healthy subjects [1]. The label links that long half-life to the up-to-36-hour effect and notes that interactions can last longer than with sildenafil, whose half-life is about 4 hours [1].
Pharmacokinetics that shape the mechanism
Tadalafil is absorbed with a median peak concentration at 2 hours, with a range of 30 minutes to 6 hours [1]. Food does not change the rate or extent of absorption, and the label says the tablet may be taken with or without food [1]. It is 94% protein bound and has a volume of distribution of about 63 L [1].
It is metabolized mainly by CYP3A4, with mean oral clearance of 2.5 L/h [1]. Once-daily dosing reaches steady state within 5 days, with exposure about 1.6-fold that of a single dose [1]. The label notes that once-daily use provides continuous plasma tadalafil levels [1].
The same CYP3A4 pathway explains why some drugs change exposure. Ketoconazole 400 mg daily raised tadalafil 20 mg AUC by 312%, and ritonavir 200 mg twice daily raised it by 124% [1]. Rifampin 600 mg daily cut tadalafil 10 mg AUC by 88% and peak concentration by 46% [1]. The Cialis label gives dose limits with potent CYP3A4 inhibitors and says no dose adjustment is needed with inducers, though once-daily efficacy may fall by an unknown amount [1].
Selectivity: PDE5, PDE6, and PDE11
Tadalafil is not equally potent across phosphodiesterases. The Cialis label reports that in vitro tadalafil is about 700-fold more potent for PDE5 than for PDE6, the retinal enzyme [1]. It is 14-fold more potent for PDE5 than PDE11A1 and 40-fold more potent than PDE11A4 [1]. The label states that PDE11 inhibition has no defined clinical consequence in humans [1].
Blood pressure, nitrates, and alpha-blockers
PDE5 inhibition has a small systemic blood-pressure effect. In healthy volunteers, tadalafil 20 mg lowered supine blood pressure by a mean maximal 1.6/0.8 mm Hg compared with placebo [1].
Organic nitrates are contraindicated with Cialis because tadalafil potentiated the hypotensive effect of nitrates in clinical pharmacology studies [1]. In a life-threatening situation where a nitrate is medically necessary, the label says at least 48 hours should elapse after the last Cialis dose before a nitrate is considered, and then only under close medical supervision with hemodynamic monitoring [1]. That is an emergency rule, not a routine timing schedule. In a 150-man crossover study the nitroglycerin interaction was significant at every time point up to and including 24 hours [7]. The label says it was not seen by most measures at 48 hours, although a few more tadalafil men had larger blood pressure falls [1].
Alpha-blockers also lower blood pressure. The label says the effect is additive and symptomatic hypotension such as fainting can occur [1]. For ED, the label says a man should be stable on the alpha-blocker first and start tadalafil at the lowest dose [1]. For BPH, the label does not recommend combining Cialis with an alpha-blocker because efficacy has not been adequately studied and blood pressure can fall [1].
Neither Cialis nor Adcirca has a boxed warning [1].
What about BPH and PAH?
PDE5 is also in prostate, bladder, and pulmonary vascular smooth muscle [1]. For BPH, the Cialis label states that the mechanism for reducing symptoms has not been established [1]. A 2011 review described possible effects on smooth muscle relaxation, smooth muscle and endothelial cell proliferation, nerve activity, and tissue perfusion, but concluded that the precise mechanism remains unclear [8]. In a 12-week trial, tadalafil 5 mg once daily (n=161) changed the International Prostate Symptom Score by -5.6 versus -3.6 with placebo (n=164; P=0.004), with no significant improvement in peak urine flow or post-void residual volume [9].
For pulmonary arterial hypertension, tadalafil is a different product at a different dose. Adcirca is tadalafil 20 mg tablets approved for PAH [10]. In the 405-patient PHIRST trial, only tadalafil 40 mg met the prespecified significance level for 6-minute walk distance; the mean placebo-corrected effect was 33 m (95% CI 15 to 50 m) [11].
Why response can be incomplete
The label says ED and BPH should be evaluated for underlying causes [1]. Drug exposure can also matter. Rifampin reduced tadalafil AUC by 88%, and the label says the reduced exposure can be anticipated to decrease the efficacy of once-daily Cialis, though the magnitude is unknown [1].
