GLP-1 Receptor Agonists: Titration and Tapering Algorithms
At a glance
- Class / GLP-1 receptor agonists; every current US product is prescription-only [1][2]
- Why titration / labels start low and step up over weeks to limit GI side effects [1][2]
- Wegovy injection / 0.25 mg weekly for 4 weeks, then a step every 4 weeks; usual maintenance 2.4 mg [1]
- Wegovy tablets / 1.5 mg daily for 30 days, then a step every 30 days; maintenance 25 mg [1]
- Foundayo tablets / 0.8 mg daily, steps at least 30 days apart; maximum 17.2 mg [2]
- Nausea in weight trials / 44% with Wegovy 2.4 mg vs 16% placebo; 35% with Foundayo 17.2 mg vs 10% [1][2]
- Stopping / STEP 4: -7.9% weight on continued semaglutide vs +6.9% after switch to placebo at 48 weeks [3]
Why the labels start low
Every product in the class is labeled to begin below its maintenance dose and increase stepwise rather than start at the target dose [1][2].
The purpose the labels give is tolerability. In the Foundayo weight-management trials, nausea, vomiting and diarrhea were more frequent during the dose-escalation period and fell over time [2].
Wegovy injection is taken once weekly, while Wegovy tablets and Foundayo are taken once daily [1][2].
Wegovy: the labeled schedule
For the injection, the label starts at 0.25 mg once weekly for 4 weeks, then titrates every 4 weeks toward the maintenance dose; the usual maintenance dose is 2.4 mg once weekly [1]. In STEP 4, the escalation ran 16 weeks before 4 weeks at the 2.4 mg maintenance dose [3].
For Wegovy tablets, the label starts at 1.5 mg once daily for 30 days and titrates every 30 days to a recommended maintenance dose of 25 mg once daily [1]. Tablets are taken on an empty stomach with up to 4 ounces of water, at least 30 minutes before food, other drinks or other oral medicines [1].
Foundayo: the labeled schedule
The starting dose is 0.8 mg once daily. After at least 30 days the dose goes to 2.5 mg, and after at least another 30 days to 5.5 mg. From there it may rise to 9 mg, 14.5 mg or 17.2 mg, after at least 30 days at each level, based on treatment response and tolerability; the maximum is 17.2 mg once daily [2]. Tablets are swallowed whole, one per day, with or without food [2].
How much GI trouble the trials reported
In weight-management trials, nausea was reported by 44% of people taking Wegovy 2.4 mg versus 16% taking placebo, diarrhea by 30% versus 16%, vomiting by 24% versus 6% and constipation by 24% versus 11%; 6.8% on Wegovy and 3.2% on placebo stopped treatment because of adverse reactions [1]. These figures cover the trials as a whole, not only the weeks of dose escalation [1].
Foundayo 17.2 mg produced nausea in 35% versus 10% on placebo, diarrhea in 25% versus 11%, vomiting in 24% versus 4% and constipation in 24% versus 9%; 8% overall stopped for adverse reactions versus 3% on placebo, and 10% at the 17.2 mg dose [2]. Severe gastrointestinal reactions were reported in about 3% of orforglipron-treated patients versus 1% on placebo [2]. The labels note that rates from different trials are not directly comparable [1][2].
What the labels flag during and after a dose increase
- Before the first dose: a personal or family history of medullary thyroid carcinoma or MEN 2 is a contraindication for these products [1][2].
- Pancreatitis: the labels say to discontinue if pancreatitis is suspected [1][2].
- Kidney injury from dehydration: the Wegovy and Foundayo labels recommend monitoring kidney function when nausea, vomiting or diarrhea could cause dehydration, especially when starting or increasing Foundayo [1][2].
- Hypoglycemia with insulin or a sulfonylurea: the labels say a lower insulin or sulfonylurea dose may be needed when starting Wegovy or Foundayo because the combination raises the risk of low blood sugar [1][2]. In the Foundayo trial in type 2 diabetes, glucose below 54 mg/dL occurred in 7% of patients taking a sulfonylurea versus 0.5% of those who were not [2].
- Heart rate: the Wegovy label reports mean resting heart rate increases of 1 to 4 bpm, and the Foundayo label reports 4 to 5 bpm versus 0.5 bpm on placebo [1][2].
- Procedures: pulmonary aspiration during general anesthesia or deep sedation has been reported with GLP-1 receptor agonists, and patients are instructed to tell the surgical team about planned procedures [1][2].
- Foundayo dose handling: the maximum dose is 9 mg daily with a strong CYP3A4 inhibitor, strong CYP3A4 inducers are to be avoided, and simvastatin is not to exceed 20 mg daily, according to the Foundayo label [2].
Stopping: what the labels say, and what STEP 4 showed
The Wegovy and Foundayo dosing sections describe step-up schedules and do not describe a step-down schedule for stopping [1][2].
The label instructions for stopping are tied to specific events. The Wegovy label says to discontinue if pancreatitis is suspected and if a serious hypersensitivity reaction occurs, and for weight reduction or cardiovascular risk reduction to discontinue when pregnancy is recognized and at least 2 months before a planned pregnancy [1]. The Foundayo label says to discontinue if pancreatitis is suspected or a hypersensitivity reaction occurs, and to discontinue when pregnancy is recognized [2].
STEP 4 measured what stopping does to weight. In STEP 4, 902 adults entered the semaglutide run-in; 803 reached 2.4 mg weekly and were randomized after 20 weeks to continue semaglutide (535) or switch to placebo (268) for 48 weeks [3]. Participants switched directly to placebo after 20 weeks of semaglutide treatment; over the next 48 weeks, they regained 6.9% of body weight, while those who continued semaglutide lost another 7.9% [3].
