Enclomiphene Citrate Monitoring Schedule: Labs and Exams You Need
At a glance
- Approval status / No FDA-approved product contains enclomiphene [1]
- Trial lab timing / Morning total testosterone at baseline and week 6; hormones and semen at 12 to 16 weeks [2][3]
- Regulator concerns / Hematocrit, PSA, estradiol, cardiac and thromboembolic events, ocular changes [1]
- Dose used in phase 3 / 12.5 mg daily, raised to 25 mg at week 6 if morning testosterone stayed below 300 ng/dL [2]
- Sperm monitoring / Concentration fell 50% or more in 14.2% on enclomiphene vs 2.6% on placebo in ZA-301 [2]
- Clomiphene label / Lists liver disease as a contraindication and warns of visual symptoms; it covers the clomiphene mixture, not enclomiphene [4]
- Long-term safety data / Not available; EMA refused authorisation partly over clot risk [1][5]
Baseline before the first dose
The phase 3 studies enrolled men with a morning total testosterone of 300 ng/dL or lower on two occasions 2 to 10 days apart and low or normal LH, below 9.4 IU/L [3]. The Endocrine Society's 2018 guideline on testosterone therapy recommends measuring fasting morning total testosterone with an accurate assay, repeating it to confirm, adding a free testosterone when the total is near the lower limit of normal or SHBG is altered, and then looking for the cause of the deficiency [6].
The phase 2 program in men measured a broader panel: total testosterone, estradiol, FSH, LH, SHBG, TSH, prolactin and IGF-1 [7].
Week 6: the dose-titration checkpoint
The phase 3 protocols started enclomiphene at 12.5 mg once daily and raised it to 25 mg daily at week 6 if morning testosterone was still below 300 ng/dL [2]. No FDA-approved dose exists; a 6-week phase 2 dose-finding trial tested 6.25, 12.5 and 25 mg daily [8].
What the trials measured at 12 to 16 weeks
The two 16-week phase 3 studies compared enclomiphene with AndroGel 1.62% and placebo, drawing hormone levels at baseline and after 16 weeks and collecting semen samples twice at enrollment and twice at the end [3]. Total testosterone rose in every treatment group; LH and FSH rose with enclomiphene and fell with the gel; sperm concentration stayed in the normal range on enclomiphene while spermatogenesis fell markedly on testosterone gel [3].
The main readout was average 24-hour testosterone falling within 300 to 1040 ng/dL. In ZA-301, 78.8% of men on enclomiphene reached that range (95% CI 70.3 to 85.3, n=113) [2]. EMA's refusal noted that the studies raised testosterone but did not examine whether symptoms improved, and FDA's own guidance says semen-parameter changes alone are not enough to establish efficacy [1][5].
What regulators flagged
FDA's 2022 review listed cardiac and thromboembolic events, including pulmonary embolism and deep vein thrombosis in a safety study, raised estradiol, raised PSA, raised hematocrit, and the absence of long-term safety data [1]. EMA noted a slightly higher rate of cardiac and thromboembolic events than with testosterone and cataracts in nonclinical studies, and wanted ocular monitoring [1].
The closest comparison data on those lab values come from clomiphene, not enclomiphene: in 77 men taking 50 mg daily or every other day, mean total testosterone rose by about 200 ng/dL while mean PSA and hematocrit (49%, 95% CI 41 to 53) stayed within normal ranges over a mean follow-up of 358 days [9]. In a separate series of 46 men on clomiphene for more than a year, testosterone was 228 ng/dL at baseline and 612, 562 and 582 ng/dL at years one, two and three, with no adverse events reported by any patient [10].
For enclomiphene itself, a retrospective review of 66 men who took clomiphene and later enclomiphene found estradiol changed by +17.50 pg/mL on clomiphene and -5.92 pg/mL on enclomiphene (P=0.001), with fewer side effects on enclomiphene (odds ratio 0.18, 95% CI 0.07 to 0.44) [11]. That is observational data, not a monitoring protocol.
Liver and vision: what the clomiphene label warns
The detailed warnings for this drug family come from the oral clomiphene label, which covers clomiphene citrate in women [4]. It lists liver disease or a history of liver dysfunction as a contraindication, and postmarketing reports include increased transaminases and hepatitis [4].
The same label reports blurred vision, spots or flashes (scintillating scotomata), which become more frequent with higher doses or longer courses and are usually reversible, although prolonged and irreversible visual disturbances have been reported [4]. Visual symptoms were recorded in 1.5% of patients in the label's clinical study table (n=8029) [4]. The label also says there are no adequate, well-controlled studies showing clomiphene is effective for male infertility, and that testicular tumors and gynecomastia have been reported in men taking clomiphene, with cause and effect not established [4].
Semen analysis and fertility
The 16-week phase 3 studies collected semen at the start and end of treatment, and enclomiphene maintained sperm concentration in the normal range while testosterone gel markedly reduced spermatogenesis [3]. In ZA-301, sperm concentration fell by 50% or more in 14.2% of men on enclomiphene versus 2.6% on placebo [2]. The 2026 British Society of Sexual Medicine position statement calls enclomiphene promising for fertility preservation while noting the evidence comes from small, short-term studies [12].
Side effects to track between visits
Headache, hot flushes, nausea and muscle spasm were the most frequent side effects of enclomiphene in the phase 3 and EMA data, and reasons for stopping included blurred vision, muscle spasm, headache and aggression [13]. In ZA-302, headache occurred in 7 of 112 men on 12.5 mg versus 1 of 47 on placebo, and nausea in 4 of 112 versus 3 of 47 [13]. In a 12-man phase 2 study, testosterone fell back to pre-treatment values one month after the drug was stopped [7].
