How to Safely Stop Enclomiphene Citrate: A Step-by-Step Discontinuation Guide

At a glance

  • Approval status / Enclomiphene is not FDA-approved, and no FDA-approved enclomiphene product has a label with stopping instructions [1]
  • Studied doses / 12.5 mg daily, with uptitration to 25 mg in phase 3 protocols [3]
  • Half-life / About 10 hours [1]
  • Taper schedule / None described in the trial reports
  • Post-stop testosterone / Returned to pretreatment values at one month in a 12-man study [2]
  • Common side effects / Headache, hot flushes, nausea, muscle spasm [4]
  • Reasons for stopping in trials / Blurred vision, muscle spasm, headache, aggression [4]
  • Long-term safety / No long-term data; FDA listed cardiac and thromboembolic events [1]

Is there a step-by-step discontinuation protocol?

Enclomiphene has no FDA-approved product or FDA-approved product label with stopping instructions [1]. Compounded enclomiphene products are not FDA-approved and have not been reviewed by FDA for safety, effectiveness or quality [5]. The British Society of Sexual Medicine position statement notes that long-term efficacy and safety data are lacking [6].

Phase 3 protocols used 12.5 mg once daily, with uptitration to 25 mg at week 6 if morning testosterone stayed below 300 ng/dL [3].

What happens to testosterone after stopping?

A study of 12 men previously treated with topical testosterone compared enclomiphene with testosterone gel [2]. After treatment stopped, total testosterone decreased one month posttreatment to pretreatment values [2].

Enclomiphene is absorbed rapidly, with peak levels 2 to 3 hours after a dose and a half-life of about 10 hours [1].

What did trials report about stopping for side effects?

In phase 3 and EMA data, the most frequent side effects were headache, hot flushes, nausea and muscle spasm. Reasons for stopping included blurred vision, muscle spasm, headache and aggression [4]. In ZA-302, headache occurred in 7 of 112 men on 12.5 mg and 1 of 47 on placebo; nausea occurred in 4 of 112 and 3 of 47 [4].

FDA's 2022 review listed cardiac and thromboembolic events, raised estradiol, raised PSA and raised hematocrit, and noted no long-term safety data [1].

Is enclomiphene the same as clomiphene?

No. Clomiphene citrate is a mixture of enclomiphene (trans) and zuclomiphene (cis); the USP monograph gives enclomiphene content of clomiphene as 50 to 70 percent [1]. Clomiphene is FDA-approved for ovulatory dysfunction in women desiring pregnancy. Its label has no male indication and says there are no adequate or well-controlled studies that demonstrate effectiveness in male infertility [7].

The Clomid label states that zuclomiphene has a longer half-life than enclomiphene and that detectable zuclomiphene levels persisted longer than a month [7]. That label covers clomiphene citrate, not enclomiphene.

What is not known

The 12-man study did not compare stopping methods [2]. It does not describe an alternate-day plan, a post-stop lab timetable, a restart threshold or a withdrawal syndrome.

Frequently asked questions

Can enclomiphene be stopped abruptly?
No trial compared abrupt stopping with a taper. The only post-stopping data come from a 12-man study that did not compare stopping methods [2].
How long does testosterone take to return after stopping?
One 12-man study found testosterone fell back to pretreatment values one month after stopping [2].
What side effects led men to stop in trials?
Reasons for stopping included blurred vision, muscle spasm, headache and aggression [4]. The most frequent side effects were headache, hot flushes, nausea and muscle spasm [4].
Does enclomiphene cause withdrawal?
No withdrawal syndrome is described in the trial reports [2][4].
