Long-Term Maintenance on GLP-1 Medications: Stopping, Tapering, Weight Regain, and Switching Drugs
At a glance
- Stopping semaglutide after a 20-week run-in / placebo switch: +6.9% body weight over 48 weeks vs -7.9% on continued semaglutide [1]
- Gap between continuing and stopping / 14.8 percentage points [1]
- Two-year trial (STEP 5) / -15.2% at week 104 vs -2.6% on placebo, 304 adults, analyzed regardless of treatment discontinuation [4]
- Usual Wegovy injection maintenance dose / 2.4 mg weekly, reached by stepping up every 4 weeks; Wegovy tablets: 25 mg daily [2]
- Foundayo (oral orforglipron) dosing / 0.8 mg start, at least 30 days between steps, maximum 17.2 mg [3]
- Cardiovascular events in SELECT / 6.5% vs 8.0% on placebo, hazard ratio 0.80, mean follow-up 39.8 months [5]
- Tirzepatide vs semaglutide over 72 weeks / -20.2% vs -13.7% weight, 751 adults [6]
- Two GLP-1 drugs together / not recommended on the Wegovy, Saxenda, Zepbound and Foundayo labels [2][7][8][3]
What happens to weight when a GLP-1 drug is stopped?
STEP 4 is the clearest test. Adults with obesity or overweight and no diabetes first took semaglutide for 20 weeks, including 16 weeks of dose escalation; 803 of them reached the 2.4 mg weekly maintenance dose and were then randomly assigned to keep taking semaglutide or switch to placebo for 48 more weeks. Both groups continued a lifestyle intervention. From week 20 to week 68, body weight fell another 7.9% in the semaglutide group and rose 6.9% in the placebo group, a 14.8 percentage point difference [1].
For contrast, here is what the drugs do while they are being taken. STEP 1 randomized 1,961 adults to semaglutide 2.4 mg or placebo for 68 weeks: -14.9% vs -2.4% body weight [9]. SURMOUNT-1 randomized 2,539 adults to tirzepatide 5, 10 or 15 mg or placebo for 72 weeks: -15.0%, -19.5% and -20.9% vs -3.1% [10].
What reviews say about weight regain
A review of weight maintenance describes the usual trajectory of obesity treatment: early weight loss, then a plateau, then progressive regain, with biological, behavioral and environmental factors all involved [11]. The American Association of Clinical Endocrinologists and American College of Endocrinology guidelines describe obesity as a complex, adiposity-based chronic disease [12].
Tapering and lower maintenance doses
STEP 4 tested continuing semaglutide against switching to placebo. It did not test tapering, a reduced maintenance dose, or a longer injection interval [1]. STEP 5 likewise compared semaglutide 2.4 mg with placebo [4].
What the labels contain is escalation. The Wegovy label directs a 0.25 mg weekly starting dose, with escalation to reduce the risk of gastrointestinal adverse reactions, and says that if a dose is not tolerated during escalation, delaying escalation for 4 weeks can be considered; the usual maintenance dose of Wegovy injection is 2.4 mg weekly [2]. The Foundayo (orforglipron) label starts at 0.8 mg once daily, moves to 2.5 mg and 5.5 mg after at least 30 days at each step, and allows further increases to 9, 14.5 or 17.2 mg, again to reduce GI reactions; the maximum is 17.2 mg [3].
How long is treatment meant to continue?
Both weight-management labels are written in long-term terms. Wegovy is indicated to reduce excess body weight and maintain weight reduction long term in adults and children 12 and older with obesity, or adults with overweight plus at least one weight-related condition [2]. Foundayo uses the same 'maintain weight reduction long term' wording for adults [3].
STEP 5 randomized 304 adults to semaglutide 2.4 mg or placebo for 104 weeks, with efficacy assessed regardless of treatment discontinuation, and found -15.2% vs -2.6% body weight, with at least 5% weight loss in 77.1% vs 34.4% [4].
Heart outcomes and diabetes
SELECT enrolled 17,604 adults with established cardiovascular disease and a BMI of 27 or more, none with diabetes, and followed participants for a mean of 39.8 months; mean exposure to semaglutide 2.4 mg or placebo was 34.2 months. The composite of cardiovascular death, nonfatal heart attack or nonfatal stroke occurred in 6.5% vs 8.0% (hazard ratio 0.80) [5]. SELECT compared semaglutide with placebo; it was not a trial of stopping treatment [5].
In people with type 2 diabetes, STEP 2 randomized 1,210 adults to semaglutide 2.4 mg, semaglutide 1.0 mg or placebo for 68 weeks; weight changed by -9.6% with 2.4 mg vs -3.4% with placebo [13]. STEP 4, the withdrawal trial, enrolled people without diabetes [1].
Switching from one GLP-1 drug to another
SURMOUNT-5 randomized 751 adults with obesity and no diabetes to open-label treatment for 72 weeks with the maximum tolerated dose of tirzepatide (10 or 15 mg) or semaglutide (1.7 or 2.4 mg). Weight fell 20.2% vs 13.7%, and waist circumference 18.4 cm vs 13.0 cm [6]. In type 2 diabetes, SURPASS-2 randomized 1,879 adults for 40 weeks and found HbA1c reductions of 2.01, 2.24 and 2.30 points with tirzepatide 5, 10 and 15 mg vs 1.86 points with semaglutide 1 mg, with 1.9, 3.6 and 5.5 kg more weight lost at those doses [14].
Both trials assigned one drug from the start; neither studied people changing drugs partway through treatment. Their results therefore describe the drugs as starting treatments, not the effect of a switch.
On combining agents, the Wegovy label says not to use it with another semaglutide product or any other GLP-1 receptor agonist; the Saxenda, Zepbound and Foundayo labels also say concomitant use with another GLP-1 receptor agonist is not recommended [2][7][8][3].
What the labels say to watch for during long treatment
The class warnings on the Wegovy and Foundayo labels include acute pancreatitis (with instructions to discontinue if it is suspected), severe gastrointestinal reactions with a caution against use in severe gastroparesis, acute kidney injury from dehydration, acute gallbladder disease, hypoglycemia when combined with insulin or a sulfonylurea, serious hypersensitivity, and increased heart rate [2][3].
Gastrointestinal effects are the common ones and are worst during dose escalation. In the Wegovy weight-management trials, nausea, vomiting, diarrhea and constipation occurred in 44%, 24%, 30% and 24% of patients vs 16%, 6%, 16% and 11% on placebo, and 6.8% vs 3.2% stopped treatment because of adverse reactions [2]. Mean resting heart rate rose 1 to 4 beats per minute with Wegovy [2] and 4 to 5 beats per minute with Foundayo vs 0.5 on placebo [3]. The labels say to discontinue the drug when pregnancy is recognized [2][3].
