  • Pregnancy planning / Wegovy label: stop at least 2 months before a planned pregnancy [3]

Does lowering the dose before stopping prevent weight regain?

Start with what is at stake. In STEP 1, 1,961 adults with obesity or overweight and no diabetes lost a mean 14.9% of body weight on semaglutide 2.4 mg over 68 weeks, against 2.4% on placebo [5].

The cleanest test of what happens when the drug is withdrawn is STEP 4. All 902 participants first took semaglutide for 20 weeks, titrated to 2.4 mg. Of those, 803 were then randomly assigned to continue semaglutide (535) or switch to placebo (268) for 48 more weeks, with lifestyle intervention in both groups. From week 20 to week 68 the continuing group lost a further 7.9% of body weight, while the placebo group gained 6.9% — a difference of 14.8 percentage points [1].

That trial switched people to placebo at a single point rather than stepping the dose down first. A 2026 narrative review drew on randomized withdrawal trials (including STEP 4), systematic reviews, meta-analyses and observational cohorts covering over 289,000 patients. It reports substantial weight regain, 60% to 90% within one year of stopping [2]. It describes the absence of validated tapering strategies and says that structured, multidisciplinary transition approaches combining pharmacological, behavioral and psychological interventions may reduce relapse after stopping [2].

What reverses when the drug stops

The 2026 review also reports that cardiometabolic benefits reverse in parallel with the weight. Modelling studies it cites suggest that glycaemic, blood pressure and lipid parameters return to baseline within approximately 12 months, while HbA1c normalizes within 12 to 18 months [2]. Early discontinuation (before one year) was associated with increased risks of coronary artery disease and heart failure compared with continued therapy [2].

Do the labels give a step-down schedule?

The Wegovy injection label starts at 0.25 mg once weekly for 4 weeks, then titrates every 4 weeks to a usual maintenance dose of 2.4 mg [3]. Across the class, doses start low and step up over weeks to reduce gastrointestinal side effects [3][4]. The Foundayo (orforglipron) tablet label starts at 0.8 mg once daily and moves up one dose level — 2.5 mg, 5.5 mg, 9 mg, 14.5 mg, 17.2 mg — after at least 30 days at each level, to a maximum of 17.2 mg [4]. Both dosage sections describe the climb; neither gives a schedule for coming off the drug.

What the labels do document is how closely side effects track dose. The Foundayo label says nausea, vomiting and diarrhea were reported more often during dose escalation and decreased over time [4]. In the pooled weight-management trials, nausea was reported by 26% of people on 5.5 mg, 34% on 9 mg and 35% on 17.2 mg, against 10% on placebo; 8% of people on Foundayo stopped treatment because of adverse reactions, against 3% on placebo [4]. In the Wegovy 2.4 mg trials, nausea was reported by 44% versus 16% on placebo, and 6.8% versus 3.2% discontinued for adverse reactions [3].

Planned surgery

The Wegovy and Foundayo labels both warn that pulmonary aspiration has been reported during general anesthesia or deep sedation in patients receiving GLP-1 receptor agonists who are undergoing elective surgeries or procedures. Both labels instruct patients to inform healthcare providers of any planned surgery or procedure [3][4].

Stopping for pregnancy

The Wegovy label says to discontinue Wegovy at least 2 months before a planned pregnancy because of semaglutide's long half-life, and to discontinue it when pregnancy is recognized for weight reduction or cardiovascular risk reduction [3]. Semaglutide's half-life is about 1 week [3]. The Foundayo label says to discontinue orforglipron when pregnancy is recognized [4]. Both labels state that weight loss offers no benefit during pregnancy and may cause fetal harm [3][4].

Switching from one GLP-1 drug to another

These labels say not to use the products together, but they do not specify a universal interval or protocol for switching. The new product's label provides its own starting dose and escalation schedule [3][4][7][8].

The choice between semaglutide and tirzepatide has been tested head to head. In SURMOUNT-5, 751 adults with obesity and no diabetes received the maximum tolerated dose of tirzepatide (10 or 15 mg) or semaglutide (1.7 or 2.4 mg) for 72 weeks: weight fell 20.2% versus 13.7%, and waist circumference 18.4 cm versus 13.0 cm [6]. In type 2 diabetes, SURPASS-2 randomized 1,879 adults to tirzepatide 5, 10 or 15 mg or semaglutide 1 mg for 40 weeks; HbA1c fell 2.01, 2.24 and 2.30 points versus 1.86, and weight fell 1.9, 3.6 and 5.5 kg more than with semaglutide [10]. Both were randomized comparisons of the two drugs, not studies of moving people from one drug to the other [10][6].

Lower doses and maintenance

Higher doses produced greater average weight loss in the dose-ranging trials, though these compared fixed doses and did not test stepping down. In SURMOUNT-1, 2,539 adults with obesity or overweight and no diabetes took tirzepatide 5, 10 or 15 mg or placebo for 72 weeks: weight fell 15.0%, 19.5% and 20.9% versus 3.1% [11]. In STEP 2, 1,210 adults with type 2 diabetes took semaglutide 2.4 mg, semaglutide 1.0 mg or placebo for 68 weeks; the 2.4 mg group lost 9.6% of body weight versus 3.4% on placebo [12].

The 2026 review concludes that discontinuation should be considered a high-risk clinical transition rather than a treatment endpoint, and that structured follow-up and multidisciplinary management are essential to preserve long-term cardiometabolic benefits [2].

Frequently asked questions

Is there an official taper schedule for Wegovy or Foundayo?
The Wegovy label starts at 0.25 mg once weekly for 4 weeks and titrates every 4 weeks to a usual maintenance dose of 2.4 mg [3]. The Foundayo label starts at 0.8 mg once daily and moves up one dose level after at least 30 days at each level, to a maximum of 17.2 mg [4]. Both describe escalation; neither gives a schedule for coming off the drug.
How much weight comes back after stopping semaglutide?
After stopping semaglutide in STEP 4, the placebo-switch group gained 6.9% of body weight over 48 weeks, while those who continued lost another 7.9% [1]. A 2026 review reports weight regain of 60% to 90% within one year and notes that validated tapering strategies are absent [2].
Can I switch from semaglutide to tirzepatide?
The Wegovy, Saxenda, Zepbound and Foundayo labels all say concomitant use with another GLP-1 receptor agonist is not recommended [3][7][8][4]. The starting dose and escalation come from the new product's label [3][4]. In SURMOUNT-5, 751 adults took the maximum tolerated dose of tirzepatide or semaglutide for 72 weeks and lost 20.2% versus 13.7% of body weight [6].
Do side effects improve when the dose goes down?
The Foundayo label reports that nausea, vomiting and diarrhea were more frequent during dose escalation and decreased over time [4]. In its trials, nausea was reported by 26% on 5.5 mg, 34% on 9 mg and 35% on 17.2 mg, versus 10% on placebo, but those are separate dose groups, not a test of stepping down [4]. The Wegovy 2.4 mg trials reported nausea in 44% versus 16% on placebo, and 6.8% versus 3.2% stopped treatment for adverse reactions [3].
What happens to blood sugar after a GLP-1 is stopped in type 2 diabetes?
A 2026 review cites modelling studies suggesting that glycaemic parameters return to baseline within approximately 12 months and HbA1c normalizes within 12 to 18 months after stopping [2].
How long before a planned pregnancy should semaglutide be stopped?
The Wegovy label says to discontinue at least 2 months before a planned pregnancy because of the long half-life of semaglutide, about 1 week, and to discontinue when pregnancy is recognized [3]. The Foundayo label says to discontinue orforglipron when pregnancy is recognized [4]. Both labels state that weight loss offers no benefit during pregnancy [3][4].
