Wegovy Injection Site Reactions: Diet Protocols That Help

At a glance

  • Rate in trials / 1.4% on Wegovy 2.4 mg vs 1% on placebo [1]
  • Diet in label / Reduced-calorie diet plus more physical activity [1]
  • Injection sites / Abdomen, thigh or upper arm, once weekly [1]
  • Site changes / Allowed without a dose change [1]
  • Commonest reactions / Nausea 44% vs 16% on placebo [1]
  • Serious allergy signs / Anaphylaxis and angioedema reported [1]

How often injection site reactions happen

The label's safety data for the 2.4 mg weekly injection come from three randomized, double-blind, placebo-controlled trials: 2,116 adults with obesity or overweight treated for up to 68 weeks, and 1,261 on placebo [1]. Injection site reactions were reported by 1.4% of Wegovy-treated adults and 1% of placebo-treated adults [1].

In the same table, nausea was reported by 44% versus 16%, diarrhea by 30% versus 16%, vomiting by 24% versus 6%, and constipation by 24% versus 11% [1]. Permanent discontinuation because of an adverse reaction occurred in 6.8% of Wegovy-treated patients and 3.2% of placebo patients, most often for nausea, vomiting or diarrhea [1].

Is there a diet that reduces them?

The nearest research is on dietary histamine itself. In people whose diamine oxidase activity is low, histamine-rich food and alcohol may provoke flushing, itching, urticaria and headache [2]. That work concerns swallowed histamine in people with histamine intolerance, not injected semaglutide, and we found no study that tested a low-histamine diet, fish oil or any other food or supplement against injection site reactions.

Wegovy injection is indicated in combination with a reduced-calorie diet and increased physical activity — for weight reduction, for cardiovascular risk reduction in adults with established cardiovascular disease, and for MASH with moderate to advanced fibrosis [1]. The reduced-calorie diet and increased physical activity recommended with Wegovy are not presented as treatments for injection site reactions [1].

What the label says to watch for

The label's hypersensitivity warning covers anaphylaxis and angioedema, which have been reported with Wegovy; the label says to discontinue Wegovy if a hypersensitivity reaction is suspected [1]. A prior serious hypersensitivity reaction to semaglutide or to any excipient in Wegovy is a contraindication [1].

Injection details in the label

  • The label says to inject Wegovy subcutaneously in the abdomen, thigh or upper arm, once weekly on the same day each week, at any time of day, with or without meals [1].
  • The label says the time of day and the injection site can be changed without a dose change [1].
  • The label says the injection should be inspected before each use and used only when the solution is clear, colorless and free of particles [1].

Frequently asked questions

How common are injection site reactions with Wegovy?
In the pooled weight-management trials in the Wegovy label, injection site reactions were reported by 1.4% of adults taking Wegovy 2.4 mg and 1% of those on placebo [1]. Nausea was reported by 44% versus 16% [1].
Does the Wegovy label recommend a specific diet?
The label states that Wegovy injection is used in combination with a reduced-calorie diet and increased physical activity [1].
What are the signs of a serious allergic reaction in the Wegovy label?
The label's hypersensitivity warning covers anaphylaxis and angioedema, which have been reported with Wegovy, and it says to discontinue Wegovy if a hypersensitivity reaction is suspected [1]. A prior serious hypersensitivity reaction to semaglutide or an excipient is a contraindication [1].
Can I change which site I inject?
The label says Wegovy is injected subcutaneously in the abdomen, thigh or upper arm once weekly on the same day each week, and that the time of day and injection site can be changed without a dose change [1].
What should I check before injecting Wegovy?
The label says to inspect the Wegovy injection before each dose and use it only if the solution is clear, colorless and contains no particles [1].
