Wegovy Sulfur Burps: Alternatives Without This Side Effect

At a glance

  • Label rate / Eructation reported by 7% on Wegovy 2.4 mg vs less than 1% on placebo [1]
  • What was counted / Belching; the adult table gives no odor description [1]
  • Also listed / Eructation is among the label's most common adverse reactions [1]
  • Wegovy tablet / Reaction types and frequencies were similar to the injection table [1]
  • Tirzepatide / Gastrointestinal events were the most common adverse events in its obesity trial [3]
  • Head-to-head data / 751 adults: tirzepatide -20.2% vs semaglutide -13.7% weight; GI events most common in both [2]
  • Label warnings with GI symptoms / Pancreatitis, gallbladder disease, severe GI reactions, kidney injury from fluid loss [1]

Does Wegovy really cause belching?

Yes, and the label puts a number on it. In three pooled placebo-controlled trials (2,116 adults on Wegovy 2.4 mg weekly and 1,261 on placebo, treated for up to 68 weeks), eructation was reported by 7% and less than 1% respectively [1]. Eructation also appears in the label's list of most common adverse reactions, defined as those reported in 5% or more of patients [1].

Belching sits inside a broader gastrointestinal picture. Any GI reaction occurred in 73% of people on Wegovy versus 47% on placebo, and 6.8% versus 3.2% stopped treatment because of an adverse reaction, most often nausea, vomiting or diarrhea [1].

The label counts belching, not the smell

The adult adverse reaction table reports eructation in 7% of people taking Wegovy versus less than 1% on placebo, but it does not describe the smell [1]. The label does state that Wegovy delays gastric emptying [1].

Would the Wegovy tablet change it?

Wegovy tablets are a once-daily oral form of semaglutide. In a 64-week trial of 204 adults with obesity or overweight and no diabetes, the label says the types and frequencies of reactions with the 25 mg daily tablet were similar to the injection table [1]. Whether tablets change belching specifically is not reported.

Is tirzepatide an alternative without the burps?

Tirzepatide is a glucose-dependent insulinotropic polypeptide and GLP-1 receptor agonist [3]. In SURMOUNT-1, 2,539 adults with obesity received tirzepatide 5, 10 or 15 mg or placebo for 72 weeks. Mean weight change was -20.9% at 15 mg versus -3.1% on placebo, and the most common adverse events were gastrointestinal, mostly mild to moderate and occurring primarily during dose escalation [3].

The largest direct comparison randomized 751 adults to tirzepatide (10 or 15 mg) or semaglutide (1.7 or 2.4 mg) for 72 weeks. Weight change was -20.2% versus -13.7%, and gastrointestinal events were the most common adverse events in both groups [2]. That trial compared tirzepatide with semaglutide 2.4 mg, not the 7.2 mg dose [2]. Tirzepatide produced greater average weight loss in that trial; the trial reports no belching comparison.

What the label sets out on dose and titration

Wegovy injection starts at 0.25 mg once weekly for 4 weeks, then steps up every 4 weeks (0.5 mg, 1 mg, 1.7 mg) and reaches maintenance from week 17 [1]. For weight reduction in adults the maintenance dose is 2.4 mg (recommended) or 1.7 mg [1]. The label says that if a dose is not tolerated during escalation, consider delaying escalation for 4 weeks [1].

Which symptoms matter more than burping

The label lists several gastrointestinal events that are more serious than belching:

  • Acute pancreatitis. The label describes persistent or severe abdominal pain, sometimes radiating to the back, that may or may not be accompanied by nausea or vomiting, and says to discontinue Wegovy if pancreatitis is suspected [1].
  • Gallbladder disease. Cholelithiasis occurred in 1.6% versus 0.7% on placebo and cholecystitis in 0.6% versus 0.2% in adult weight-reduction trials [1].
  • Severe gastrointestinal reactions. These occurred in 4.1% versus 0.9% on placebo, and the label says Wegovy is not recommended in patients with severe gastroparesis [1].
  • Kidney injury from volume depletion. Acute kidney injury occurred in 0.4 versus 0.2 cases per 100 patient-years, more often in people with prior kidney impairment, during dose titration, and after GI fluid loss [1].
  • Hypersensitivity. Anaphylaxis and angioedema have been reported; the label says to discontinue Wegovy if a reaction is suspected and to promptly seek medical advice [1].

Frequently asked questions

How common is belching with Wegovy?
In pooled weight-management trials, eructation was reported by 7% of adults on Wegovy 2.4 mg versus less than 1% on placebo, and it is listed among the most common adverse reactions [1].
Is the sulfur smell caused by sulfur-rich foods?
The adult adverse reaction table records belching without describing an odor, so a food cause for the smell is not something the Wegovy label addresses [1].
Is tirzepatide less likely to cause sulfur burps?
That has not been shown. In the 751-adult head-to-head trial, gastrointestinal events were the most common adverse events in both the tirzepatide and semaglutide groups, with mean weight change of -20.2% versus -13.7% [2].
Would switching to Wegovy tablets avoid it?
Wegovy tablets are a daily oral form of semaglutide. In a 64-week trial of 204 adults the label says tablet reaction types and frequencies were similar to the injection table [1].
Is a lower maintenance dose an option?
The Wegovy label lists 2.4 mg as the recommended maintenance dose for weight reduction in adults and 1.7 mg as the other maintenance option, reached by 4-week steps from 0.25 mg [1].
Which Wegovy symptoms does the label single out beyond belching?
Persistent severe abdominal pain sometimes radiating to the back, gallbladder disease, severe GI reactions and kidney injury after fluid loss, along with signs of a hypersensitivity reaction [1].
