Which semaglutide products does a price apply to?
Semaglutide is a GLP-1 receptor agonist sold by Novo Nordisk. FDA-approved products include Ozempic injection and tablets, Rybelsus tablets, and Wegovy injection and tablets [4][6][5][7]. Ozempic-branded 1.5, 4 and 9 mg tablets fall under the Rybelsus approval [6].
The approved uses differ. Ozempic is for type 2 diabetes: glycemic control, cardiovascular risk reduction in adults with type 2 diabetes and established cardiovascular disease, and kidney outcomes in adults with type 2 diabetes and chronic kidney disease. It has no weight-loss indication [1]. Wegovy injection is approved for weight management, cardiovascular risk reduction and noncirrhotic MASH [2]. Wegovy tablets are approved for adults for weight management and cardiovascular risk reduction, but carry no MASH indication [2]. Rybelsus and Ozempic tablets are for glycemic control and cardiovascular risk reduction in adults with type 2 diabetes [8].
Doses differ by form too: Wegovy injection reaches 2.4 mg weekly, or 7.2 mg in adults who tolerate 2.4 mg for at least 4 weeks and need more weight loss, while Wegovy tablets reach 25 mg daily [2]. Wegovy injections are taken weekly and Wegovy tablets daily, and a tablet dose cannot be swapped for an injection dose milligram for milligram [8][2].
Why the compounded route narrowed
FDA's enforcement discretion for compounding semaglutide injection products has ended: for state-licensed pharmacies and physicians it ended after a court denied the compounders' injunction request on 2025-04-24, and for outsourcing facilities FDA said it would not act until 2025-05-22 [3].
What remains is narrow. A product that is essentially a copy of an approved drug is allowed only if made rarely (FDA says it does not intend to act against four or fewer such prescriptions per calendar month) or if the prescriber documents a significant difference for that patient [3]. FDA says adding vitamin B12 may not change that when the route is the same and the amounts of semaglutide and B12 are each within 10% of their respective commercial products [3]. Outsourcing facilities generally cannot compound from bulk substances unless the substance is on FDA's outsourcing bulk list, and FDA says semaglutide is not on it [3]. FDA proposed on 2026-04-30 to exclude semaglutide from that list, with no final decision [9]. FDA says compounded drugs are not FDA approved and are not reviewed for safety, effectiveness or quality before marketing [10].
The bottom line
The useful comparison is the total cost of the exact product, form and dose that applies to a given person, not one program's monthly figure set against another's. The labels settle what each approved product is for [8][1][2], and FDA's 2025 and 2026 actions describe how narrow the compounded route has become [3][9].
