Compounded Semaglutide Weight Loss Plateau
At a glance
- What people call a plateau / several weeks with little or no change on the scale
- Continuing the same dose / a further 7.9% weight loss from week 20 to week 68 vs a 6.9% gain on placebo [1]
- Highest labelled dose / 2.4 mg weekly maintenance, or up to 7.2 mg in adults who tolerate 2.4 mg for at least 4 weeks and need more loss [5]
- 7.2 mg vs 2.4 mg / minus 18.7% vs minus 15.6% at 72 weeks, vs minus 3.9% on placebo [3]
- With type 2 diabetes / mean minus 9.6% on 2.4 mg vs minus 3.4% on placebo in STEP 2 [6]
- After stopping / about two-thirds of lost weight regained within a year in the STEP 1 extension [7]
- Compounded semaglutide / not FDA approved; not reviewed for safety, effectiveness or quality [4]
What the trials show about weight change over time
In STEP 4, every participant first went through a 20-week run-in with semaglutide titrated to 2.4 mg weekly, and the 803 who reached that dose had lost a mean 10.6% of body weight. They were then randomly assigned to keep taking semaglutide or switch to placebo for 48 more weeks. The continuing group lost another 7.9%; the placebo group gained 6.9% [1].
In that trial population the average weight was still falling between week 20 and week 68 on an unchanged maintenance dose. Those are group means measured at fixed timepoints; they describe the average curve, not what one person's scale does from week to week.
In SELECT, a cardiovascular outcomes trial in 17,604 adults with established cardiovascular disease and overweight or obesity but no diabetes, mean weight change at week 104 was minus 9.4% on semaglutide 2.4 mg versus minus 0.9% on placebo [8].
How much weight loss is expected on each dose
In STEP 1, 1,961 adults with obesity, or overweight plus a weight-related condition, and no diabetes took semaglutide 2.4 mg weekly or placebo for 68 weeks, both with lifestyle intervention, so the trial cannot separate the effect of any single lifestyle change. Mean weight change was minus 14.9% versus minus 2.4%; 86.4% versus 31.5% lost at least 5%, and 50.5% versus 4.9% lost at least 15% [2].
STEP UP tested a higher dose in 1,407 adults with obesity and no diabetes over 72 weeks: mean weight change was minus 18.7% on 7.2 mg, minus 15.6% on 2.4 mg and minus 3.9% on placebo [3].
With type 2 diabetes the numbers run lower. In STEP 2, 1,210 adults with type 2 diabetes and a body mass index of 27 or more lost a mean 9.6% on 2.4 mg versus 3.4% on placebo over 68 weeks [6]. The Wegovy label notes less weight loss in patients with type 2 diabetes than in those without it [5]. These averages do not show how a plateau will unfold for an individual.
The dose question: what the label actually allows
The Wegovy injection label escalates on a fixed schedule: 0.25 mg for weeks 1 to 4, 0.5 mg for weeks 5 to 8, 1 mg for weeks 9 to 12, 1.7 mg for weeks 13 to 16, then maintenance from week 17 at 2.4 mg, with 1.7 mg as an alternative maintenance dose [5].
Beyond that, the label allows a maximum of 7.2 mg weekly in adults who have tolerated 2.4 mg for at least 4 weeks and need more weight loss. Pediatric patients aged 12 and older top out at 2.4 mg [5].
In STEP UP, 7.2 mg produced 3.1 percentage points more weight loss than 2.4 mg at 72 weeks [3]. The label's adverse reaction tables show nausea in 39% on 7.2 mg, 35% on 2.4 mg and 13% on placebo; dysesthesia, an altered skin sensation such as burning or pain from touch, in 22%, 6% and 0%; and discontinuation for adverse reactions in 5%, 5% and 2% [5].
Why a compounded product adds a separate question
Compounded semaglutide is not FDA approved, and FDA does not review compounded drugs for safety, effectiveness or quality before they are marketed [4]. As of 2026-05-31, FDA had received 990 adverse event reports for compounded semaglutide and says this likely undercounts. The agency also reports dosing errors with compounded injectable semaglutide, including patients measuring the wrong dose, clinicians miscalculating, and doses above the label. It states that semaglutide sodium and semaglutide acetate salts are different active ingredients from the approved drug, with no lawful basis for use in compounding that it is aware of [4].
HealthRX.com offers compounded semaglutide by prescription.
Weight holding steady on treatment versus regain after stopping
The STEP 1 extension followed 327 participants from both groups for a year after treatment stopped. Those who had taken semaglutide had lost a mean 17.3% by week 68, regained 11.6 percentage points within a year of stopping and finished at a net 5.6% loss at week 120, with most cardiometabolic improvements reverting toward baseline [7]. That is weight change after stopping, a different situation from weight holding steady while treatment continues.
